目的 评估口服150 mg米非司酮及300 mg米非司酮在足月妊娠中促宫颈成熟效果及对新生儿的安全性观察。方法 250位宫颈Bishop评分≤5分的初产妇,随机分为三个治疗组,观察其服药前后的宫颈Bishop评分改变及产科结局。结果 150 mg组及300 mg组均具有有效的促宫颈成熟作用,150 mg组胎儿宫内窘迫发生率为12%,合并胎儿宫内窘迫而剖宫产者28.57%(10/35)。300 mg组胎儿宫内窘迫发生率为23%,合并胎儿宫内窘迫而剖宫产者52.63%(20/38)。两组间有显著性差异(P<0.05)。结论 米非司酮为受体水平的抗孕激素,它可软化宫颈、提高子宫的活性。在本研究中150 mg组米非司酮具有有效的促宫颈成熟作用,且胎儿宫内窘迫发生率相对较低。

Objectives To compare the effectiveness and safety of 150 mg and 300 mg oral mifepristone on cer-vical ripening of primigravidae. Methods 250 primigravidae at term with a Bishop score of 5 or less were randomly as-signed to one of the three treatment groups, and to observe the alterations in Bishop score after oral Mifepristone andthe outcomes of pregnancy. Results 150 mg and 300 mg oral mifepristone could improve cervical ripening. In thegroup treated with 300 mg mifepristone, the rate of fetal distress was 23% and rate of cesarean section was 52.63%(20/38). While in the group treated with 150 mg mifepristone, the rate of fetal distress was 12 % and rate of cesareansection was 28.57%(10/35). Conclusions Mifepristone, a progesterone antagonist, is known to cause softening anddilation of the early pregnant cervix and increase in uterine activity. In this study, 150 mg of mifepristone improves thecervical ripening and causes lower rate of fetal distress. (Shanghai Med J, 2000,23:486-488)
