目的 通过对正常人与弱视患者视网膜电图明视负波(PhNR)对比分析,探讨PhNR在弱视患者中的临床应用价值.方法 正常人30名60只眼(年龄5~35岁),弱视48例60只眼(5~35岁),选择蓝色背景光(亮度为25 cd/m2),刺激光为红色光(亮度2 cds/m2)下进行明视负波检查,再根据视觉发育敏感期分为<12岁和≥12岁两个年龄组,记录各种检查结果并分析.结果 (1)弱视组PhNR反应振幅均值均较正常组降低,其差异有统计学意义.(2)弱视组PhNR潜伏期和正常组相 似,其差异不具有统计学意义.(3)正常人≥12岁年龄组PhNR反应振幅均值较<12岁年龄组降低,潜伏期延迟,其差异具有统计学意义.结论 (1)弱视患者PhNR反应振幅降低,与正常组差异显著有统计学意义,提示PhNR可以作为弱视患者的一个视功能评价指标,弱视患者视网膜神经节细胞功能存在一定程度的抑制.(2)≥12岁PhNR的反应振幅较<12岁降低,潜伏期延迟,提示PhNR反应与年龄有关联.

Objective To analyze the clinical application value of Photopic Negative Response (PhNR) in amblyopia. Methods PhNR was recorded and analyzed from 60 normal eyes and 60 amblyopia eyes by using red flashes on a blue background. Results Amplitudes of PhNR in amblyopia were lower than those of the normal group,there were statistically significant differences between them.Latencies of PhNR in amblyopia were similar to normal group,there were no statistically significant differences between them.In normal group,amplitudes of PhNR of ≥12 year older age group were lower and latencies were longer compared with <12 years low group,there was statistically significant difference. Conclusions Amplitudes of PhNR in amblyopia are lower than those of the normal group,and the function of retinal ganglion cells of amblyopia is damaged.Amplitudes of PhNR decrease with age,latencies of PhNR delay with age,these suggest that PhNR is significantly associated with age.
