目的 探讨胃癌组织中信号转导及转录活化因子(Stat3)和血管内皮生长因子(VEGF)的表达,及其在胃癌的发生发展及转移中所起的作用.方法 应用免疫组织化学方法检测正常胃组织、胃癌及淋巴结转移癌组织中Stat3和VEGF的表达情况.结果 65例胃癌组织中Stat3和VEGF的表达阳性率分别为64.6%(42/65)、76.9%(50/65),均明显高于正常对照组(均为0)(P<0.05).不同组织学类型胃癌中Stat3、VEGF表达阳性率不同,依次为未分化癌>黏液癌>低分化癌>腺癌,但差异无统计学意义(P>0.05).不同分化程度和不同临床分期胃癌组织中Stat3、VEGF表达阳性率不同,低、未分化组[(77.4%(24/31)、90.3%(28/31)]明显高于高、中分化组[45.6%(21/46)、65.2%(30/40)](P<0.05);Ⅲ+Ⅳ期[78.6%(22/28)、92.9%(26/28)]明显高于Ⅰ+Ⅱ期[49.0%(24/49)、67.4%(33/49)],且差异具有统计学意义(P<0.05).Stat3与VEGF的表达与是否伴有淋巴结转移有关(P<0.01).结论 Stat3与VEGF参与了胃癌的发生与肿瘤的转移,Stat3可作为判断肿瘤预后的指标,VEGF和Stat3之间存在正相关性,与肿瘤转移、侵袭有密切联系.

Objective To detect signal transducer and activator of transcription 3(Stat3)and vascular endothelial growth factor(VEGF)expression and their roles in development and metastasis of gastric cancer.Methods Immunohistochemistry was used to detect the expression of VEGF and Stat3 in normal gastric tissue, gastric carcinoma,lymph node metastasis and VEGF expression of Stat3.Results The positive expression rate of Stat3 and VEGF in gastric carcinoma was significantly higher than that in control group[64.6%(42/65)vs 0,76.9%(50/65)vs 0,all P < 0.05].The positive expression rate of Stat3 and VEGF was different in different histological types of gastric cancer; the positive expression rate of Stat3 and VEGF in undifferentiated carcinoma patients was 90.0%,90.0%,respectively.The positive expression rates of Stat3 and VEGF in different histological grade and the different clinical stages of gastric carcinoma were different. The positive expression rates of Stat3 and VEGF in poorly differentiated group and undifferentiated group was significantly higher than that in well-differentiated group and moderately differentiated group; Ⅲ + Ⅳ period [78.6%(22/28),92.9%(26/28)]was significantly higher than the phaseⅠ + Ⅱ [49.0%(24/49),67.4%(33/49)](P < 0.05).Stat3 and VEGF expression were associated with lymph node metastasis(P < 0.01).Conclusions Stat3 and VEGF are involved in gastric carcinogenesis,tumor metastasis and cancer prognosis.VEGF and stat3 are positively correlated with tumor transfer and invasion.
