目的 探讨血清肿瘤标志物肿瘤抗原15-3( CA15-3)、细胞角蛋白19片段抗原(CYFRA21-1)、恶性肿瘤特异性生长因子(TSGF)和肿瘤抗原19-9(CA19-9)联合检测诊断乳腺癌的临床价值.方法 采用电化学发光法检测乳腺癌组(182例)患者血清CA15-3、CYFRA21-1和CAl9-9,化学比色法检测TSGF水平,并与良性病变组(80例)、正常对照组(50例)比较.结果 乳腺癌组患者血清CA15-3、CYFRA21-1、TSGF、CA19-9水平及阳性率明显高于乳腺良性病变组及正常对照组(P<0.01).CA15-3对乳腺癌的敏感性为60.4% (110/182),特异性为90.0% (45/50);CYFRA21-1敏感性为72.5%( 132/182),特异性为86.0%( 43/50);TSGF对乳腺癌的敏感性为74.7% (136/182),特异性为92.0% (46/50);CA19-9敏感性为36.3% (66/182),特异性为84.0%( 42/50);四项联合检测的敏感性高达91.8%( 167/182),特异性为84.0%( 42/50).结论 CA15-3、CYFRA21-1、TSGF、CA19-9检测对乳腺癌有一定的诊断价值,四项联合检测有明显的互补性,可明显提高诊断乳腺癌的敏感性和准确性.

목적 탐토혈청종류표지물종류항원15-3( CA15-3)、세포각단백19편단항원(CYFRA21-1)、악성종류특이성생장인자(TSGF)화종류항원19-9(CA19-9)연합검측진단유선암적림상개치.방법 채용전화학발광법검측유선암조(182례)환자혈청CA15-3、CYFRA21-1화CAl9-9,화학비색법검측TSGF수평,병여량성병변조(80례)、정상대조조(50례)비교.결과 유선암조환자혈청CA15-3、CYFRA21-1、TSGF、CA19-9수평급양성솔명현고우유선량성병변조급정상대조조(P<0.01).CA15-3대유선암적민감성위60.4% (110/182),특이성위90.0% (45/50);CYFRA21-1민감성위72.5%( 132/182),특이성위86.0%( 43/50);TSGF대유선암적민감성위74.7% (136/182),특이성위92.0% (46/50);CA19-9민감성위36.3% (66/182),특이성위84.0%( 42/50);사항연합검측적민감성고체91.8%( 167/182),특이성위84.0%( 42/50).결론 CA15-3、CYFRA21-1、TSGF、CA19-9검측대유선암유일정적진단개치,사항연합검측유명현적호보성,가명현제고진단유선암적민감성화준학성.
Objective To investigate the clinical value of the combined detection of the serum tumor marker(including CA15-3,CYFRA21-1,TSGF and CA19-9) in the diagnosis and treatment of breast cancer.Methods The serum samples of 182 patients with breast cancer were collected,and tumor markers CA15-3,CYFRA21-1,TSGF and CA19-9 were detected with electrochemiluminescence method,and TSGF was detected with chemocolorimetry method.The results were compared with 80 cases of benign breast disease and 50 normal cases.Results The serum levels of CA15-3,TSGF,OPN and CA125 of breast cancer patients were significantly higher than those of the benign breast disease and the normal cases (P < 0.01 ).The sensitivity of CA15-3 to breast cancer was 60.4% ( 110/182),with a specificity of 90.0% (45/50),the sensitivity of CYFRA21-1 was 72.5 ( 132/182 ),with a specificity of 86.0% (43/50),the sensitivity of TSGF was 74.7% ( 136/182 )with a specificity of 92.0% (46/50),and the sensitivity of CA19-9 was 36.3% (66/182),with a specificity of 84.0% (42/50).The sensitivity of the combination detection of above tumor markers was 91.8% ( 167/182 ),with a specificity of 84.0% (42/50).Conclusions CA15-3,CYFRA21-1,TSGF and CA19-9 have certain diagnostic value in breast cancer complementarily,and the combined detection can significantly improve the sensitivity and accuracy of breast cancer diagnosis.
