Diabetes education Effect evaluation
目的 研究糖尿病教育在糖尿病控制中的作用。方法 用随机单盲对照法,将147例糖尿病患者分为强化教育组(A),一般教育组(B),A组在一般的糖尿病教育后,针对每一例患者进行个体强化教育。B组则仅进行一般的糖尿病教育。每例患者均于教育前、教育后3个月末、教育后6个月末调查糖尿病知识积分(DKN),精神症状自评积分(SCL-90),每日摄取热量、每周主动运动治疗次数及每周血(尿)糖监测次数,并检测糖化血红蛋白(HbA1c),血脂(TC、TG),了解患者对糖尿病的认识、自控水平、心理精神障碍及糖、脂代谢改善情况,进行组内教育前后及同期组间比较。结果 A组在教育后3个月末和6个月末,自控能力明显提高,DKN、HbA1c、TC、TG等指标改善明显优于B组(P<0.01或P<0.05)。在SCL-90的9组症状群中,A组在教育后均有明显改善,且优于同期B组(P<0.01或P<0.05)。结论 糖尿病知识强化教育,能更有效提高患者自控水平,增强对糖尿病的认识,改善糖、脂代谢水平。同时能有效改善精神和心理障碍,提高患者生命质量。

목적 연구당뇨병교육재당뇨병공제중적작용。방법 용수궤단맹대조법,장147례당뇨병환자분위강화교육조(A),일반교육조(B),A조재일반적당뇨병교육후,침대매일례환자진행개체강화교육。B조칙부진행일반적당뇨병교육。매례환자균우교육전、교육후3개월말、교육후6개월말조사당뇨병지식적분(DKN),정신증상자평적분(SCL-90),매일섭취열량、매주주동운동치료차수급매주혈(뇨)당감측차수,병검측당화혈홍단백(HbA1c),혈지(TC、TG),료해환자대당뇨병적인식、자공수평、심리정신장애급당、지대사개선정황,진행조내교육전후급동기조간비교。결과 A조재교육후3개월말화6개월말,자공능력명현제고,DKN、HbA1c、TC、TG등지표개선명현우우B조(P<0.01혹P<0.05)。재SCL-90적9조증상군중,A조재교육후균유명현개선,차우우동기B조(P<0.01혹P<0.05)。결론 당뇨병지식강화교육,능경유효제고환자자공수평,증강대당뇨병적인식,개선당、지대사수평。동시능유효개선정신화심리장애,제고환자생명질량。
Objective To evaluate the effects of the intensive diabetes education on diabetes control.Methods 147 diabetics (type 1 and type 2) were randomly allocated into two groups:Group A(intensive education group ) and Group B( conventional education group).Each patient from Group A was given personal intensive knowledge education after conventional knowledge education.The patients from Group B were only given conventional diabetes education.Investigations into the diabetic knowledge (DKN),symptom check list 90 (SCL 90),daily taken calories,numbers of exercise each week and of blood glucose,HbA1c,T Chl and TG monitoring each week were conducted before the education,three months and six months after education.The results were evaluated and compared before and after education within the group and between the two groups.Results The patients from Group A had obviously better self controlling abilities at the end of three months and six months education.Futhermore,the improvement of indexes such as DKN,HbA1c,TC,and TG in Groups A were better than those in Group B( P <0.05 or P <0.01).In the nine subgroups of SCL 90,the patients from Group A were clearly improved and better than those of the simultaneous period from Group B.( P <0.01 or P <0.05).Conclusion Intensive diabetes education will effectively enhance diabetes self controlling levels,enlarge their perceptions about diabetes and improve their glucose and fat metabolism.It can also effectively change the patients’ spiritual states,thus improve the life quality of diabetes.
