目的 探讨基质金属蛋白酶2 (MMP-2)和MMP-9及基质金属蛋白酶组织抑制物1(TIMP-1)和TIMP-2在非黑色素性皮肤癌(NMSC)中的表达及其临床意义.方法 病理证实的NMSC患者组织标本68例,其中鳞状细胞癌(SCC)36例,基底细胞癌(BCC)32例,取肿瘤中心组织和边缘癌组织;同时,以30例正常皮肤组织为对照组.应用免疫组织化学法和逆转录聚合酶链反应( RT-PCR)法,检测各组中MMP-2、MMP-9、TIMP-1和TIMP-2的表达情况.结果 对照组中MMP-2和MMP-9的阳性率分别为30.0%和36.7%,MMP-2和MMP-9的表达水平分别为57.216±12.785和59.318±13.262,均显著低于SCC组和BCC组的中心及边缘癌组织(P<0.01);对照组中TIMP-1和TIMP-2的阳性率分别为96.7%和100%,TIMP-1和TIMP-2的表达水平分别为121.738±25.516和122.612±25.964,均显著高于SCC组和BCC组的中心及边缘癌组织(P<0.01).SCC组中心及边缘癌组织中MMP-2和MMP-9的表达水平显著高于BCC组中心及边缘癌组织,而TIMP-1和TIMP-2表达水平显著低于BCC组中心及边缘癌组织(P<0.01).SCC组和BCC组边缘癌组织中,MMP-2的表达水平分别为118.438±28.517和101.672±19.137,MMP-9的表达水平分别为115.816±26.416和99.731±19.058; SCC组和BCC组中心癌组织中,MMP-2的表达水平分别为97.197±18.359和87.825±17.351,MMP-9的表达水平分别为95.732±18.157和85.182±16.973,SCC组和BCC组边缘癌组织中MMP-2和MMP-9的表达水平明显高于中心癌组织(P<0.01).SCC组和BCC组边缘癌组织中,TIMP-1的表达水平分别为82.872±16.285和91.653±17.197,TIMP-2的表达水平分别为83.157±16.174和91.926±17.283;SCC组和BCC组中心癌组织中,TIMP-1的表达水平分别为93.561±17.037和103.356±19.738,TIMP-2的表达水平分别为94.019±17.046和104.191±19.887,SCC组和BCC组边缘癌组织中TIMP-1和TIMP-2的表达水平明显低于中心癌组织(P<0.01).结论 MMP-2、MMP-9及其抑制因子TIMP-1、TIMP-2与NMSC的发生、发展密切相关,可能在NMSC的侵袭与转移中有重要作用.

Objective To investigate the expression and significance of matrix metalloproteinases (MMP-2,MMP-9) and tissue inhibitor of matrix metalloproteianse (TIMP-2,TIMP-1 ) in non-melanoma skin cancer (NMSC).Methods Thirty six patients with squamous cell carcinoma (SCC) and 32 patients with basal cell carcinoma (BCC),confirmed by pathology,were selected,and 30 cases of normal skin were selected as control.The expression of MMP-2,MMP-9,TIMP-1 and TIMP-2 in all samples were examined by immunohistochemistry and reverse transcription polymerase chain reaction (RT-PCR).The expression rate,expression intensity and expression level of each factor were recorded.The results were compared between the groups.Results The expression rates of MMP-2 and MMP-9 in the control group were 30.0% and 36.7%,the expression levels of MMP-2 and MMP-9 in the control group were 57.216 ± 12.785 and 59.318 ±13.262,all significantly lower than those in the tumor edge and center of the SCC and BCC groups (P <0.01 ).The expression rates of TIMP-1 and TIMP-2 in the control group were 96.7% and 100%,their expression levels were 121.738 ± 25.516 and 122.612 ± 25.964,all significantly higher than those in the SCC and BCC groups ( P <0.01 ).The expression levels of MMP-2 and MMP-9 in the tumor center and edge of SCC group were significantly higher than those in the corresponding parts of the BCC group,while the expression levels of TIMP-1 and TIMP-2 were significantly lower than those in the BCC group (P <0.01 ).The expression levels of MMP-2 and MMP-9 in the tumor edge of the SCC and BCC groups were significantly higher titan those in the tumor centers (P <0.01 ),while the expression levels of TIMP-1and TIMP-2 were significantly lower than those in the tumor centers ( P <0.01 ).Conclusion MMP-2,MMP-9 and TIMP-2,TIMP-1 may play an important role in the development,progression,invasion and metastasis of nonmelanoma skin cancer.
