目的探讨主动脉内球囊反搏(IABP)在老年急性心肌梗死(AMI)合并心源性休克患者中的应用及护理.方法 2003年至2005年2月,收治ICU的14例AMI合并心源性休克患者,年龄61～72岁,发病3～6 h内应用IABP及综合性治疗.根据老年患者的特点,加强IABP参数、管道监测和护理、生命体征监测、呼吸机辅助呼吸、有效供氧,做好心理护理和基础护理.结果 14例患者应用天数5～12 d,其中9例血流动力学稳定后行支架植入术,3例病情平稳后行冠状动脉搭桥术,2例因经济问题放弃治疗而死亡.结论 AMI合并心源性休克,及时采用IABP和综合治疗,采取相应的护理措施,可改善心脏功能,是进一步治疗的关键.

Objective Discuss The IBAP'S application and nursing in the gerontal AMI complicated cardiac shock.Methods From the 2003 to February 2005,cured 14 patients with AMI complicated cardiac shock,aged between 61～72,invasion in 3～6 h,giving the IABP and complex treatment.In cording to the garontal characteristic,enhanced the IABP parameter,tube monitoring and nursing,vital sign monitoring,assisted mechanical ventilation,effective oxygen supply,making psychological nursing,basic nursing.Results 14 patients curing 5～12 days,9 cases whit stable hemodynamic had stent implantation,3 Cases with stable condition had the coronary bypass,2 cases gave up treatment and died because of the economic problem.Conclusion In AMI complicated cardiac shock,giving the IABP and complex treatment immediately,with corresponding nursing provision,can improve the heart function,is the key of the advanced treatment.
