目的探讨肝细胞性肝癌(HCC)合并不同模式乙型肝炎血清标志物的意义,为HCC的普查、预防和临床诊断提供依据.方法筛选HCC患者标本840例,使用美国雅培公司提供的ARCHITECT i2000SR化学发光免疫定量分析仪进行乙型肝炎血清标志物的检测,根据乙型肝炎血清标志物不同模式分成14组.结果 1)在不同模式乙型肝炎血清标志物中HCC检出率:A组HBsAg(+)、抗-HBe(+)、抗-HBe(+)"小三阳"模式为59.29%(498/840);B组HBsAg(+)、HBeAg(+)、抗-HBc(+)"大三阳"模式为20.48%(172/840),其他12组中均有HCC检出,与A组和B组比较差异有统计学意义(P<0.01).2)在HCC患者中乙型肝炎血清标志物的检出率为抗-HBc>HBsAg>抗-HBe>HBeAg>抗-HBs.结论 1)对于既往感染乙型病毒性肝炎或感染后恢复以及亚临床感染的患者均有HCC发生的可能,除了高危人群,以上人群均需重视定期体检.2)HBsAg(+)和抗-HBc(+)与HCC的发生均存在相关性,而抗-HBc比HBsAg等其他乙型肝炎血清标志物更具敏感性.

Objective To investigate different models of hepatitis B serum markers in hepatocellular carcinoma(Hepatocelluar Carciroma,HCC),provide a basis for HCC survey prevention and clinical diagnosis.Methods Screening of 840 samples of HCC patients were test by Abbott ARCHITECT i2000SR from the American.The samples were divided into 14 groups of different models based on serum markers of hepatitis B.Group A is HBsAg(+)、HBeAb(+)、HBcAb(+).Group B is HBsAg(+)、HBeAg (+)、HBcAb(+).Results 1)The HCC detection rate of Group A is 59.29%(498/840);The HCC detection rate of Group B is 20.48%(172/840),there were significant differences(P<0.01)for the other 12 groups were detected in HCC between group A and group B.2)The detection rate of HBV serum markers in HCC patients HBcAb>HBsAg>HBeAb>HBeAg>HBsAb.Conclusion 1)For the patients with the past hepatitis B infection or after infection,as well as the other Clinical Infections,the occurrence of HCC was possible,besides high-risk groups,other patients need also regular medical check-ups;2)HBsAg(+)and HBcAb(+)are associated with the occurrence of HCC.but HBcAb is more sensitive than HBsAg and other serum markers of hepatitis B.
