目的:探讨慢性肾脏疾病患者外周血血小板粘附分子P选择素(CD62P)及淋巴细胞粘附分子(ICAM1)CD54的变化及其与病变程度及疗效的关系。方法:应用荧光单克隆抗体分子探针CD62P及CD54分别对36例不同的慢性肾脏疾病患者外周血血小板和淋巴细胞进行了免疫学标记,并用流式细胞技术定量检测。结果:①慢性肾脏疾病患者外周血小板CD62P表达量均不同程度增加,外周血淋巴细胞CD54也显著性增加(P<0.05或P<0.01);②随着病程的好转,上述分子表达量逐渐下降,有显著性意义(P均<0.05),但仍然高于正常对照组。结论:外周血血小板粘附分子CD62P及淋巴细胞粘附分子CD54在慢性肾脏疾病发病机制及病情进展中起重要作用,其值高低与病变程度及疗效密切相关,观察外周血CD62P和CD54在该疾病中的变化有相当重要的临床价值。

Objective:To explore the changes in the expression of Pselectin in peripheral platelets and intercellular adhesion molecule1(ICAM1) in peripheral blood lymphocytes and their relationship with the severity of diseases and clinical outcome in patients with chronic glomerular diseases.Methods:Thirtysix patients with different chronic glomerular diseases were included in the study.Peripheral blood samples were collected.CD62P and CD54 were labeled using fluorescent monoclonal antibodies and then determined by flow cytometry.Results:① Both the expression of Pselectin in platelets and that of ICAM1 in peripheral lymphocytes increased in patients with chronic glomerular diseases as compared with the normal controls (P<0.05 or P<0.01,respectively);②As the patient's condition turns better,the expression of Pselectin and ICAM1 decreased significantly (all P<0.05),but still higher than those in healthy control.Conclusions:Pselectin and ICAM1 in peripheral blood platelets and lymphocytes play pivotal roles in the pathogenesis and development of chronic glomerular diseases,the levels of which were well correlated with the severity of the disease.The changes in the peripheral blood CD62P and CD54 were thus of important prognostic value in such patients.
