目的:了解慢性阻塞性肺疾病(COPD)患者脂质代谢特点及其与病情的相关性。方法:测定151例COPD患者(慢支炎肺气肿组65例,慢性肺心病组86例)及62例正常人血清甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-ch)、载脂蛋白A1(APO A1)、载脂蛋白B100(APO B100)的水平。结果:慢支炎肺气肿组HDL-ch降低、APO B100升高;慢性肺心病组TG、TC、HDL-ch、APO A1均降低,APO B100升高,APO A1/APO B100比值降低。与对照组比较,差异均有显著性(P<0.01)。慢性肺心病组APO A1水平较慢支炎肺气肿组下降更为明显(P<0.01)。结论:COPD患者存在脂质代谢异常。在肺心病阶段脂质代谢紊乱较慢支炎肺气肿阶段更为明显。

목적:료해만성조새성폐질병(COPD)환자지질대사특점급기여병정적상관성。방법:측정151례COPD환자(만지염폐기종조65례,만성폐심병조86례)급62례정상인혈청감유삼지(TG)、총담고순(TC)、고밀도지단백담고순(HDL-ch)、재지단백A1(APO A1)、재지단백B100(APO B100)적수평。결과:만지염폐기종조HDL-ch강저、APO B100승고;만성폐심병조TG、TC、HDL-ch、APO A1균강저,APO B100승고,APO A1/APO B100비치강저。여대조조비교,차이균유현저성(P<0.01)。만성폐심병조APO A1수평교만지염폐기종조하강경위명현(P<0.01)。결론:COPD환자존재지질대사이상。재폐심병계단지질대사문란교만지염폐기종계단경위명현。
Objective: To understand the correlation between the lipid metabolism and clinical features in patients with COPD(chronic obstrutcive pulmonary disaease).Methods: Serum triglyceride(TG), total cholesterol(TC),high density lipoprotein-cholesterol(HDL-ch),apolipoprotein A1(APO A1) and apolipoprotein B100(APO B100) were measured in 151 patients with COPD and 62 normal persons as control.Results:The mean values of HDL-ch of the patients with chronic bronchitis were lower than those of the controls,but APO B100 was higher.The mean value of APO B100 of the patients with chronic pulmanary heart disease was highe than that of the controls, but TG, TC,HDL-ch,APO A1 and APO A1/APO B100 were lower.There were statisticatly significant differences between them and controls(P<0.01). The mean values of serum APO A1 in patients with chronic pulmonary heart disease were sighificantly lower than that in the chronic bronchitis patients(P<0.01).Conclusions:There was an abnormality of lipid metabolism in the patients with COPD and the lipid metabolic disorder in the patients with chronic pulmonary heart disease was more significant than that in the chronic bronchitis patiants.
