PDA%postoperative hypertension%prevention and treatment%thoracic epidural analgesia
Objective To investigate the effects of thoracic epidural analgesia on postoperative hypertension in patients with PDA. Methods 48 cases of PDA were randomly divided into NTEA group (n=22) and TEA group (n=26). General anesthesia was used for NTEA group, and intramuscular injection of pethidine 1 mg/kg was used when postoperative pain occurs; general anesthesia combined with thoracic epidural anesthesia were used for TEA group, and thoracic epidural analgesia was used for postoperative analgesia. BP,HR,RR, SpO2, EKG were monitored during and after operation, angiotenic was used when the BP elevated more than 20% compared with before operation. Results Analgesia effect on TEA group was better than that of NTEA group (P< 0.05), also postoperative BP was significantly higher than that of before operation in NTEA group(P< 0.01), and 18 cases need nitroglycerin or sodium nitroprusside therapy; there was no significantly increase of BP in TEA group, only 2 cases need nitroglycerin therapy. Conclusion TEA had an excellent effect of postoperative pain relief and blocked sympathetic nerve in patients with PDA, so it could reduce postoperative hypertension effectively.
