经非手术治疗效果欠佳的颈椎间盘突出症患者128例,在影像引导下经颈椎旁后入路注射胶原蛋白酶600 U,治疗后3月、6月、1年随访,有效随访77例,失访51例.男性38例,女性39例,年龄28～62岁,病史5 d～20年.采用改良MacNab标准和视觉模拟评分法(VAS)评估患者的疗效.患者治疗后1年按改良MacNab标准评价其疗效优良率达86%.治疗前VAS评分＜6分的患者治疗后疼痛未见复发,而治疗前VAS评分＞6分的56例严重疼痛患者,治疗后3月、6月、1年VAS评分逐渐降低(P＜0.05).治疗后1年随访未发现与治疗有关的并发症.颈椎旁后入路注射胶原酶600U治疗颈椎间盘突出症患者安全可靠且远期疗效好.

One hundred and twenty-eight patients with herniation of cervical intervertebral disc which failed to respond to the palliative lxeatment were treated by cervical paravertebral injection of collagenage 600 U via posterior approach under image guidance.The patients were followed up for one year.The follow-up was considered to be complete in 77 patients.The effectiveness of the treatment wag assessed using modified MacNab standard and VAS.The long term complications were also recorded.According to modified MacNab standard the effectiveness of cervical paravertebral injection of collagenage was rated as excellent/good in 86%of the patients.Patients with VAS score ＜6 did not recur aftertreatment while in patients with VAS score＞6,the VAS Score was graduaUy decreasing during the 3,6 and 12 months after treatment.No comphcation related to the collngenase injection was found.Cervical paravertebral collagenage injection via posterior approach is a safe and effective treatment for herniation of cervical intervertebral disc.
