目的报告2例阴茎离断伤再植成功的经验和体会,探讨阴茎离断伤后再植的方法及注意要点.方法应用显微外科手段对阴茎离断伤进行再植.结果本组2例患者术后再植阴茎均完全成活,术后随访4～6年,2例患者阴茎痛触觉恢复较好,再植成活的阴茎勃起功能恢复,能进行正常性生活,无尿道狭窄等并发症.结论熟悉相应的解剖,术前、术中尽量缩短阴茎缺血时间,高质量吻合阴茎重要血管、神经,术后加强护理、合理用药,是阴茎再植成功的关键.
목적 보고2례음경리단상재식성공적경험화체회,탐토음경리단상후재식적방법급주의요점.방법 응용현미외과수단대음경리단상진행재식.결과 본조2례환자술후재식음경균완전성활,술후수방4～6년,2례환자음경통촉각회복교호,재식성활적음경발기공능회복,능진행정상성생활,무뇨도협착등병발증.결론 숙실상응적해부,술전、술중진량축단음경결혈시간,고질량문합음경중요혈관、신경,술후가강호리、합리용약,시음경재식성공적관건.
Objective To report experiences on 2 successful cases of penile replantation, and explore method and key points for attention in penile amputation after replantation.Methods The application of microsurgical instruments was used in penile amputation for replantation.Results 2 cases after replantation of the penis survived completely,postoperative follow-up for 4 ～ 6 years,two patients with penile pain tactile recovered well,as well as the replantation of penis erectile function recovery,being able to engage in normal sexual life,without any complications such as urethral stricture.Conclusion To be familiar with the relevant anatomy,try to shorten the penile ischemia time during preoperative and intraoperative operation,high quality of anastomosis of penile vascular and nerve,postoperative care and rational drug use,are all the keys for successful penis replantation.
