目的探讨微创闭合复位经皮交叉克氏针固定治疗小儿完全移位的肱骨髁上骨折的可行性与临床效果.方法选择78例小儿肱骨髁上骨折完全移位的病例,在C臂X光机透视下,三维手法复位满意后,选择1.5～2.0mm克氏针经皮交叉内固定,适当功能练习,术后3周摄片,愈合良好则拔出克氏针.结果术后摄片达解剖复位者65例,全部病例随访6～24个月,平均12个月,按李稔生对肱骨髁上骨折的疗效评定标准,优68例,良6例,可3例,差1例.结论微创闭合复位经皮克氏针固定治疗小儿完全移位肱骨髁上骨折损伤小、固定牢固、并发症少、肘关节功能恢复良好,临床疗效确切,值得推广应用.

Objective To discuss the feasibility and clinical effect of treatment complete displaced supracondylar humerus fractures in children by minimally invasive closed reduction and cross percutaneous pinning with Kirschner wire. Methods 78 cases of Gartland type Ⅲ were treated with internal fixation with 2 Kirschner wires under"C"arm fluoroscopy and closed reduction in three dimension. Proper exercises were taken after 3 weeks and the wire was taken out after the clinical data with X-ray. Results 65 cases were found X-ray film to see anatomical reduction, 78 cases had been followed up for 6-24 months with the average of 12 months, according to Lee Jin-sheng's the humerus fractures on the assessment criteria, excellent in 68 cases, good in 6 cases , fair in 3 cases and poor in 1 case .Conclusion This method of minimally invasive surgical treatment provides minimal damage, reliable fixation , rare complication, satisfied elbow joint motion and the perfect effect with advantages that should be applied.
