目的:探讨64层螺旋CT多平面重建(MPR)技术诊断孤立型细支气管肺泡癌(bronchioloalveolar carcinoma,BAC)的价值.材料和方法:分析经病理证实的孤立型BAC 30例的CT表现,利用三维成像软件行多平面重建,比较横断面图像与MPR图像在孤立型BAC基本征象方面的显示价值.结果:孤立型BAC 30例,MPR重建图像显示清晰,与横断面图像比较,在显示"磨玻璃征"、"深分叶征"、"环晕征"、"毛刺征"、胸膜凹陷征、血管集束征、细支气管充气征方面MPR显著优于常规横断位,MPR并能减少"空泡征"的假阳性.结论:MPR重建方法较简单易行,较常规横断位扫描能更准确地显示孤立型BAC的形态特征,对孤立型BAC诊断具有重要价值.

Purpose To explore the value of muti-planar reconstruction (MPR) with 64-slices spiral CT in diagnosing solitary bronchioloalveolar carcinoma (BAC).Materials and Methods The CT manifestations of pathologically-proved BAC were analyzed. MPR images were obtained with 3D reconstruction software, and the findings on the MPR images were compared with primary axial images.Results For all 30 cases of BAC, MPR images were significantly superior to conventional axial images in the aspect of demonstrating ground glass opacity, deep lobulation, halo sign, spiculation, pleural indentation, vascular convergence sign and air-bronchogram. Meanwhile, the false vacuole sign was reduced on MPR images. Conclusions MPR with spiral CT, easily performed, can show the morphology of BAC more precisely than routine axial images, which could benefit the diagnosis of solitary BAC.
