目的探讨应用比目鱼肌肌瓣修复小腿胫前中、下段软组织缺损合并胫骨开放性骨折和(或)骨髓炎的有效性.方法 2007年5月至2011年12月,应用远侧端为全部、近侧端为内侧部的蒂在上端的比目鱼肌肌瓣修复8例小腿胫中、下段软组织缺损合并胫骨开放性骨折和(或)骨髓炎患者,缺损面积为8 cmu×6 cm～12 cm×10cm,其中5例合并慢性骨髓炎,3例合并开放性骨折并发急性骨髓炎.比目鱼肌肌瓣表面以大张中厚皮片覆盖,供瓣区直接闭合.结果 8例患者所移植的比目鱼肌肌瓣,以及覆盖其上的大张中厚皮片均全部成活,且肌肉组织覆盖可靠、骨折愈合,术后随访2～54个月,小腿外形和功能均满意.结论比目鱼肌肌瓣是修复小腿胫前中、下段软组织缺损合并胫骨开放性骨折和(或)骨髓炎的有效和可行的方法,供区损伤小,修复效果好.
목적 탐토응용비목어기기판수복소퇴경전중、하단연조직결손합병경골개방성골절화(혹)골수염적유효성.방법 2007년5월지2011년12월,응용원측단위전부、근측단위내측부적체재상단적비목어기기판수복8례소퇴경중、하단연조직결손합병경골개방성골절화(혹)골수염환자,결손면적위8 cmu×6 cm～12 cm×10cm,기중5례합병만성골수염,3례합병개방성골절병발급성골수염.비목어기기판표면이대장중후피편복개,공판구직접폐합.결과 8례환자소이식적비목어기기판,이급복개기상적대장중후피편균전부성활,차기육조직복개가고、골절유합,술후수방2～54개월,소퇴외형화공능균만의.결론 비목어기기판시수복소퇴경전중、하단연조직결손합병경골개방성골절화(혹)골수염적유효화가행적방법,공구손상소,수복효과호.
Objective To investigate the efficacy of surgical method with the soleus muscle flap for repairing the soft-tissue defects in the middle and distal thirds of the leg,combined with open tibial fracture and/or osteomyelitis.Methods From May 2007 to December 2011,8 cases with soft-tissue defects in the middle and distal thirds of the leg were treated with soleus muscle flaps,including 5 cases with chronic osteomyelitis and 3 cases with open tibial fracture and acute osteomyelitis.The defects size ranged from 8 cm× 6 cm to 12 cm × 10 cm.The surface of muscle flaps was covered by split-thickness skin graft.The defects at the donor site were closed directly.Results All the muscle flaps and skin grafts survived completely and fracture was healed.The patients were followed up for 2 to 54 months with satisfactory cosmetic and functional results.Conclusions The soleus muscle flap is an effective and feasible method for reconstruction of the soft-tissue defects in the middle and distal thirds of the leg,combined with open tibial fracture and/or osteomyelitis.The morbidity at donor sites is also minor.
