目的探讨联合检测胸水D-二聚体(D-dimer,D-D)和癌胚抗原(CEA)对恶性胸腔积液的诊断价值.方法回顾性分析36例结核性胸腔积液和32例恶性胸腔积液患者的胸水D-二聚体和癌胚抗原的检测资料.结果恶性胸腔积液患者胸水的D-二聚体和癌胚抗原的含量均显著高于良性胸腔积液组[(57.43±2.41)mg/L比(12.63±3.23)mg/L,(73.2±6.43)μg/L比(4.15±2.35)μg/L,P＜0.05)}联合检测能提高诊断恶性胸腔积液的敏感度(84.3%)和准确度(86.7%).结论联合检测胸水D-二聚体和癌胚抗原对鉴别良、恶性胸腔积液具有一定的临床价值.

Objective To explore the diagnostic value of malignant perfusions of D-D and CEA in pleural fluid.Methods The clinical data of the levels of D-D, CEA in pleural effusion of 36 hospitalized patients with benign pleural effusions and 32 patients with malignant perfusion were analyzed retrospectively. Rusults malignant pleural effusion of D-dimer and carcinoembryonic antigen levels were significantly higher than benign pleural effusion group [( 57.43 ± 2.41 ) mg/L vs ( 12.63 ± 3.23 ) mg/L;( 73.2 ± 6.43 ) μ g/L vs ( 4.15 ± 2.35 ) μ g/L, P＜ 0.05 )],Joint detection can improve the sensitivity of diagnosing malignant pleural effusion( 84.3％ )and accuracy ( 86.7％ )significantly higher than those in benign perfusions. Conclusiong Combined measurement of D-D ,CEA in pleural fluid has a preferable clinical value in defferential diagnosis of benign and malignant malignant perfusiongs.
