目的比较半髋关节置换与动力髋螺钉内固定术(DHS)治疗老年人粗隆间骨折的临床效果. 方法选择老年粗隆问骨折患者35例,随机分为A组、B组.A组20例采用DHS髋动力钢板治疗老年粗隆间骨折.B组15例加长柄人工双极股骨头置换治疗老年粗隆间骨折.随访12至48个月,平均24个月.对所有患者手术前和手术后髋关节功能进行Harris评分.统计分析比较A、B两组的治疗效果. 结果 A组并发症:泌尿系感染3例,肺部感染2例,内固定松动1例,髋内翻3例.B组并发症:泌尿系感染1例,术后疼痛1例.B组患者术后并发症少于A组(P=0.049);A、B两组手术前功能进行Harris评分无显著差异(t=0.527,P=0.602),术后功能进行Harris评分无显著差异(t=0.602,P=0.551). 结论半髋关节置换患者可以早期下地,可以减少术后并发症.

Objective To evaluate the effect of artificial femoral head replacement and DHS for the treatment of elderly patients with intertrochanteric fracture.Methods Group A:20 elderly patients with intertrochanteric fracture were treated by DHS.Group B:15 elderly patients with intertrochanteric fracture were treated by artificial femoral head replacement.Followed up for 12 to 48 months,average 24 months.All cases were evaluated by Harris scores preoperatively and at last follow-up.Then the curative effect was compared.Results Group B can reduce complication(P=0.049).There was no significant difference in preoperative Harris scores(t=0.527,P=0.602).There was no significant difference in postoperative Harris scores(t=0.602,P=0.551).Conclusions The treatment of artificial femoral head replacement in elderly patients with intertrochanteric fracture can reduce complication.There was no significant difference between the results of treatment with artificial femoral head replacement and DHS.
