目的 探讨导乐陪伴分娩对妊娠产程及结局的影响.方法 将80例顺产的初产妇随机分为对照组和观察组,每组各40例.对照组采用产房顺产常规护理方式,观察组采取导乐陪伴分娩的护理方式,观察两组初产妇的产程时间、自然分娩率、新生儿Apgar评分、产后24h出血情况.结果 观察组的第一、二、三产程时间和总产程时间分别是(425.68±173.59) min、(41.28±23.69) min、(6.13±3.44) min、(481.29±159.73)min,明显短于对照组的(534.46±162.57) min、(61.24±34.61) min、(7.81±3.47) min、(586.42±165.37) min,P＜0.05.观察组的自然分娩率和新生儿Apgar评分分别是85.00％和9.76±0.43,明显高于对照组的67.50％和9.01±0.39,P＜0.05.观察组产后24h出血量为(121.47±34.57) ml,明显少于对照组的(152.26±48.13) ml,P＜0.05.结论 导乐陪伴分娩明显缩短产程,改善妊娠结局.

Objective To investigate the effect of doula support during childbirth on pregnancy labor and outcomes.Methods A total of 80 patients in the primiparae natural delivery were randomly divided into control group and observation group,40 patients in each group.Control group was given usual care in delivery room,the observation group received doula support during childbirth.Labor time,natural delivery rate,Apgar scores and postpartum 24 h hemorrhage were observed in two groups.Results The first,second and third stage of labor time and the total process time in the observation group that were (425.68 ± 173.59) min,(41.28 ± 23.69) min,(6.13 ± 3.44)min,(481.29 ± 159.73) min were significantly shorter than those _ (534.46 ± 162.57) min,(61.24± 34.61) min,(7.81 ± 3.47)min,(586.42 ± 165.37) min} in the control group,P ＜0.05.Natural birth rate and neonatal Apgar scores of the observation group were 85.00％ and 9.76 ± 0.43,which were significantly higher than those (67.50％ and 9.01 ± 0.39) in the control group,P ＜0.05.Postpartum24h hemorrhage was ( 121.47 ± 34.57) ml in the observation group,that was significantly less than that { ( 152.26 ± 48.13) ml}in the control group,P ＜ 0.05.Conclusions Doula support during childbirth could significantly shorten labor time and improve the pregnancy outcome.
