目的 研究活血散结中药外用治疗乳腺增生病的作用机制.方法 60只老年大鼠随机分为对照组、模型组、散结乳癖组、三苯氧胺组、活血散结中药小、中、大剂量组.通过雌孕激素联合造模方法,形成老年乳腺增生病模型鼠,造模同时给予不同药物进行干预.观察治疗前后模型鼠体重、血液流变学、乳房血流动力学、左侧第2只乳头高度及直径、血清激素等方面变化.结果 大鼠经造模并用药干预后,第2周内出现模型组及各用药组大鼠体重增长幅度较对照组明显降低(P<0.01),第3周后恢复正常.大鼠乳腺微循环灌注量,模型组和各用药组低于对照组(P<0.01);经药物干预治疗后,各用药组微循环灌注量高于模型组(P<0.01),活血散结中药中、大剂量组高于散结乳癖组(P<0.05).活血散结中药外用能够影响模型大鼠的血液流变学参数,改善血液的高黏高凝状态.各药物组均能明显抑制雌孕激素造模所引起的大鼠乳头高度的变化(P<0.01),活血散结中药大、中剂量组效果优于小剂量组和散结乳癖组(P<0.05).除活血散结中药小剂量组外,其余各药物干预组均能明显抑制雌孕激素造模所引起的大鼠乳头直径的变化(P<0.01) ,三苯氧胺组疗效优于其他各组(P<0.05);活血散结中药中剂量组和散结乳癖组相比有明显差异(P<0.05).活血散结中药能显著降低模型大鼠的雌激素(E_2)、泌乳素(PRL)、卵泡生成素(FSH) 水平,升高孕激素(P)、血清睾酮(T)和黄体生成素(LH)水平(P<0.05).各药物均能显著降低模型大鼠乳腺增生的程度(P<0.01),活血散结中药大剂量组优于散结乳癖组(P<0.05),与三苯氧胺组无显著性差异(P>0.05).结论 活血散结中药组方外用可改善乳房部血液循环,抑制腺体增生,促进纤维组织吸收,有效调整体内激素水平,对乳腺增生病有良好疗效.

목적 연구활혈산결중약외용치료유선증생병적작용궤제.방법 60지노년대서수궤분위대조조、모형조、산결유벽조、삼분양알조、활혈산결중약소、중、대제량조.통과자잉격소연합조모방법,형성노년유선증생병모형서,조모동시급여불동약물진행간예.관찰치료전후모형서체중、혈액류변학、유방혈류동역학、좌측제2지유두고도급직경、혈청격소등방면변화.결과 대서경조모병용약간예후,제2주내출현모형조급각용약조대서체중증장폭도교대조조명현강저(P<0.01),제3주후회복정상.대서유선미순배관주량,모형조화각용약조저우대조조(P<0.01);경약물간예치료후,각용약조미순배관주량고우모형조(P<0.01),활혈산결중약중、대제량조고우산결유벽조(P<0.05).활혈산결중약외용능구영향모형대서적혈액류변학삼수,개선혈액적고점고응상태.각약물조균능명현억제자잉격소조모소인기적대서유두고도적변화(P<0.01),활혈산결중약대、중제량조효과우우소제량조화산결유벽조(P<0.05).제활혈산결중약소제량조외,기여각약물간예조균능명현억제자잉격소조모소인기적대서유두직경적변화(P<0.01) ,삼분양알조료효우우기타각조(P<0.05);활혈산결중약중제량조화산결유벽조상비유명현차이(P<0.05).활혈산결중약능현저강저모형대서적자격소(E_2)、비유소(PRL)、란포생성소(FSH) 수평,승고잉격소(P)、혈청고동(T)화황체생성소(LH)수평(P<0.05).각약물균능현저강저모형대서유선증생적정도(P<0.01),활혈산결중약대제량조우우산결유벽조(P<0.05),여삼분양알조무현저성차이(P>0.05).결론 활혈산결중약조방외용가개선유방부혈액순배,억제선체증생,촉진섬유조직흡수,유효조정체내격소수평,대유선증생병유량호료효.
Objective To investigate the mechanism of Huoxuesanjie external use for hyperplasia of mammary gland. Methods 60 aged rats were randomly divided into control, model, Sanjierubi emplastrum, tamoxifen paste, Huoxuesanjie traditional Chinese estraderm elements extracting solution small, middle and large dose groups. Hyperplasia mammary gland model rats were made by the union of estrogen and progestational hormone. At the same time of making model, the rats were given different durg. The body weight, hemorheology, breast hemodynamic, height and diameter of the left second ripple and serum hormone level were detected before and after treatment. Results Compared with control group, the body weight increasing degree decreased in model and each treatment groups on second week(P <0. 01) and regain the normal level on the third week. The mammary gland circulation perfusion in control group was higher than that of model and each treatment groups ( P < 0. 01). After drug interference, the mammary gland circulation perfusion in each treatment group was higher than that of model group (P <0. 01 ) , and the mammary gland circulation perfusion in Huoxuesanjie middle and large dose group was higher than that of sanjierubi group(P <0. 05). Huoxuesanjie could improve the model rat's blood high viscosity and high-coagulate state. The change of ripple height in each treatment group could be inhibited (P <0. 01) and the effect of large and middle dose of Huoxuesanjie were better than that of small dose of Huoxuesanjie and sanjierubi ( P < 0. 05 ). Except small dose of Huoxuesanjie, the other drug could inhibite the change of ripple diameter ( P <0. 01) , and tamoxifen was better than other drugs and there was obvious difference between middle Huoxuesanjie and sanjierubi ( P < 0. 05 ) . Huoxuesanjie could efficiently decreased the level of estrogen, prolactin ( PRL), follicle-stimulating hormone ( FSH) and increase the level of progestational hormone, testosterone, and luteinizing hormone (LH) (P < 0. 05). Each drug could decrease the hyperplasia of mammary gland degree (P <0. 01) , the effect of large dose of Huoxuesanjie was better than that of sanjierubi (P <0. 05) and there was no difference between the large dose of Huoxuesanjie and tamoxifen (P > 0. 05). Conclusions Huoxuesanjie external use has a better therapeutic effect on hyperplasia of mammary gland by improving blood circulation, inhibiting gland hyperplasia, stimulating fibrous-tissue absorption and regulating hormone level.
