目的:应用苄基二甲基十四烷氯化铵(benzyldimethyltetradecylammoniumchloride,BAC)建立犬下食管括约肌(LES)无神经动物模型,研究一氧化氮(NO)对LES压力的作用。方法:将BAC环周注入犬LES,对照组注入等量生理盐水,均于注射前及注射后6周测定LES压力;并观察L-精氨酸、D-精氨酸、硝普钠及一氧化氮合酶(NOS)抑制剂N-硝基-L-精氨酸(L-NNA)对LES压力的影响;此外还测定了两组犬LES中NO含量和NOS活性。结果:BAC处理组LES压力[(42.43±4.19)mmHg,1mmHg=0.133kPa]显著高于对照组[(22.71±5.19)mmHg]。L-精氨酸可使对照组LES压力降低;L-NNA使其增高,但对BAC处理组LES压力均无影响。硝普钠可降低两组犬LES压力。对照组LES中NO为(6.058±2.067)μmol/g,NOS为(1.458±0.146)U/mg;而BAC处理组NO为(1.797±0.873)μmol/g,NOS为(0.463±0.039)U/mg,均较对照组显著降低(P<0.01)。结论:BAC可使犬LES压力增高,其机制可能与LES局部NO减少有关。

목적:응용변기이갑기십사완록화안(benzyldimethyltetradecylammoniumchloride,BAC)건립견하식관괄약기(LES)무신경동물모형,연구일양화담(NO)대LES압력적작용。방법:장BAC배주주입견LES,대조조주입등량생리염수,균우주사전급주사후6주측정LES압력;병관찰L-정안산、D-정안산、초보납급일양화담합매(NOS)억제제N-초기-L-정안산(L-NNA)대LES압력적영향;차외환측정료량조견LES중NO함량화NOS활성。결과:BAC처리조LES압력[(42.43±4.19)mmHg,1mmHg=0.133kPa]현저고우대조조[(22.71±5.19)mmHg]。L-정안산가사대조조LES압력강저;L-NNA사기증고,단대BAC처리조LES압력균무영향。초보납가강저량조견LES압력。대조조LES중NO위(6.058±2.067)μmol/g,NOS위(1.458±0.146)U/mg;이BAC처리조NO위(1.797±0.873)μmol/g,NOS위(0.463±0.039)U/mg,균교대조조현저강저(P<0.01)。결론:BAC가사견LES압력증고,기궤제가능여LES국부NO감소유관。
Objective: To develop a denervated dog model in the loweresophageal sphincter(LES) and to investigate the role of nitric oxide(NO). Methods: Benzyldimethyltetradecyl ammonium chloride(BAC) was endoscopically injected circumferentially into the LES in 10 adult dogs and saline was injected in 10 adult dogs as control. Pre-injection and 6 weeks after injection, manometry was performed. With L-arginine, D-arginine, N-nitro-L-arginine(L-NNA) and sodium nitroprusside(SNP), pharmacological responses of the LES pressure(LESP) were observed and the content of NO and the activity of nitric oxide synthase (NOS) in LES were determined. Results: LESP in BAC-treated dogs was significantly higher than that in control [(42.43±4.19) mmHg vs (22.71±5.19) mmHg]. L-arginine significantly decreased and L-NNA increased LESP in control, but L-NNA had no effects on BAC-treated dogs. SNP reduced LES pressure in all dogs. The content of NO and the activity of NOS in LES of BAC-treated dogs were markedly lower than that in control [NO:(1.797±0.873) μmol/g vs (6.058±2.067) μmol/g), NOS: (0.463±0.039) U/mg vs (1.458±0.146) U/mg]. Conclusion: Injection of BAC into LES can increase LESP, and the elevated pressure of LES in dog may be closely related to the loss of NO in LES.
