目的 探讨不同程度原发性甲状腺功能减退(甲减)对肌酶谱的影响,观察甲减患者激素替代治疗后肌酶谱的动态变化.方法 收集原发性甲减患者68例,其中亚临床甲减组21例,中度甲减组15例,重度甲减组32例,另选取39例健康体检者作为对照组.分别在初诊和激素替代治疗后1个月、2个月检测各组血清游离T3(FT3)、游离T4(FT4)、促甲状腺激素(TSH)、肌酸激酶(CK)、CK-MB、乳酸脱氢酶(LDH)、α-羟丁酸脱氢酶(HBD)、天门冬氨酸氨基转移酶(AST)、肌钙蛋白(anTL)水平.结果 原发性甲减患者的CK、CK-MB、LDH、HBD、AST水平均高于对照组,以CK升高最明显,各酶均与FT3、FT4呈负相关,与TSH呈正相关.不同程度甲减对肌酶的影响不同,中度甲减组与亚甲减组之间比较有增高趋势,但差异没有统计学意义(P>0.05),重度甲减组与前两组相比,除CK-MB、HBD外,其他肌酶均升高,差异均有统计学意义(P<0.05).甲减患者经激素替代治疗后4~6周肌酶恢复正常.结论 甲减时肌酶水平随甲减程度加重而升高,经激素替代治疗后肌酶逐渐恢复.

목적 탐토불동정도원발성갑상선공능감퇴(갑감)대기매보적영향,관찰갑감환자격소체대치료후기매보적동태변화.방법 수집원발성갑감환자68례,기중아림상갑감조21례,중도갑감조15례,중도갑감조32례,령선취39례건강체검자작위대조조.분별재초진화격소체대치료후1개월、2개월검측각조혈청유리T3(FT3)、유리T4(FT4)、촉갑상선격소(TSH)、기산격매(CK)、CK-MB、유산탈경매(LDH)、α-간정산탈경매(HBD)、천문동안산안기전이매(AST)、기개단백(anTL)수평.결과 원발성갑감환자적CK、CK-MB、LDH、HBD、AST수평균고우대조조,이CK승고최명현,각매균여FT3、FT4정부상관,여TSH정정상관.불동정도갑감대기매적영향불동,중도갑감조여아갑감조지간비교유증고추세,단차이몰유통계학의의(P>0.05),중도갑감조여전량조상비,제CK-MB、HBD외,기타기매균승고,차이균유통계학의의(P<0.05).갑감환자경격소체대치료후4~6주기매회복정상.결론 갑감시기매수평수갑감정도가중이승고,경격소체대치료후기매축점회복.
Objective To discuss the effect of different degrees of hypothyroidism on the muscle enzymes and observe the dynamic changes after hormone replacement therapy in patients with hypothyroidism.Methods Collected 68 cases of patients with hypothyroidism,including 21 patients in subclinical hypothyroidism group, 15 patients in moderate hypothyroidism group,32 cases in severe hypothyroidism group, and 39 cases of healthy as control group. To test the levels of serum free triiodothyronine ( FT3 ), free thyroxine ( FT4 ), thyroid stimulating hormone ( TSH ), creatine kinase (CK), creatine kinase-MB ( CK-MB ), lactate dehydrogenase (LDH) and α-hydroxybutyrate dehydrogenase isoenzyme (HBD), aspartate aminotransferase (AST) ,troponin (anTL) in the first visit, one month and two month after hormone replacement therapy.Results The levels of CK,CK-MB, LDH,HBD, AST in patients with primary hypothyroidism group were higher than those in the control group, especially the CK level. The enzymes were negatively related to FT3 and FT4,and positively correlated with TSH. There was no significant differences in enzyme levels between subclinical hypothyroidism and moderate hypothyroidism group( P >0.05). Compared with subclinical hypothyroidism and moderate hypothyroidism group, the enzyme levels in severe hypothyroidism group have significant differences(P <0.05 ). After hormone replacement therapy,all the muscle enzymes returned to normal after 4 to 6 weeks. Conclusion Enzymes level increases with degrees of hypothyroidism. With the hormone replacement therapy, muscle enzymes improves gradually.
