目的 对内科患者下呼吸道感染病原菌的构成及耐药情况进行分析,指导临床合理用药.方法 收集2008年1~12月诊断为下呼吸道感染的内科住院患者的痰标本进行分离培养,并用全自动微生物鉴定和药敏分析仪对培养出的致病菌进行药敏试验,记录和统计试验结果.结果 在526例痰标本中共分离出15种218株病原菌,其中9种主要病原菌的分布情况是:流感嗜血杆菌20株,副流感嗜血杆菌30株,大肠埃希菌12株,肺炎克雷伯菌40株,铜绿假单胞菌38株,金黄色葡萄球菌14株,肺炎链球菌10株,真菌22株,不动杆菌15株;耐药情况为:金黄色葡萄球菌对10种抗菌药物的总耐药率为61.0%,真菌对7种抗菌药物的耐药率为7.7%.结论 下呼吸道感染患者的病原菌种类较多,革兰阴性菌感染占大多数,大肠埃希菌和铜绿假单胞菌的耐药率较高.

목적 대내과환자하호흡도감염병원균적구성급내약정황진행분석,지도림상합리용약.방법 수집2008년1~12월진단위하호흡도감염적내과주원환자적담표본진행분리배양,병용전자동미생물감정화약민분석의대배양출적치병균진행약민시험,기록화통계시험결과.결과 재526례담표본중공분리출15충218주병원균,기중9충주요병원균적분포정황시:류감기혈간균20주,부류감기혈간균30주,대장애희균12주,폐염극뢰백균40주,동록가단포균38주,금황색포도구균14주,폐염련구균10주,진균22주,불동간균15주;내약정황위:금황색포도구균대10충항균약물적총내약솔위61.0%,진균대7충항균약물적내약솔위7.7%.결론 하호흡도감염환자적병원균충류교다,혁란음성균감염점대다수,대장애희균화동록가단포균적내약솔교고.
Objective To analyze sputum-cultivated pathogenic bacterium distribution and drug resistance ratio to the inpatients suffered from lower respiratory tract infection in our hospital, and to guide the rational drug administration in clinic.Methods To collect the inpatients' Sputum specimens from January 2008 to December 2008,conduct automatic microorganism identification and drug sensitivity test analysis system to process the specimens, obtain the isolated pathogenic bacterium progress drug sensitivity test, and take down the records and statistic test results.Results Among 526 sputum examples, 13 species or 218 strains pathogenic bacterium were isolated. And 9 of 13 species pathogenic bacterium distributed were as follow:there were 20 strains H.haemolyticus,30 strains H.parahaemolyticus,12 strains E. coli,40 strains K. pneumoniae,38 strains P.aeruginosa,14 strains S.aureus,10 strains S.pneumoniae,15 strains A.baumannii and 22 strains Fungus etc;the complexion of drug resistance suggested the total drug resistance of the S.aureus anti 10 kind antibiotic was 61.0% and fungus anti 7 kind antibiotic was 7.7%.Conclusion The species of pathogenic bacterium were various.Gram negative bacteria were the main pathogenic bacteria involved in respiratory tract infection. The drug resistance was high at E. coli and P.aeruginosa.
