目的探讨宁夏全科医学培训工作中存在的问题并提出对策.方法采用两阶段调查方法.第一阶段,于2008年8-10月对宁夏全科医学培训中心、卫生厅、卫生局及社区卫生服务机构的有关领导进行访谈;第二阶段,采用分层整群抽样的方法从宁夏全区5个地级市中随机抽取20家社区卫生服务站和4家社区卫生服务中心,于2008年11月-2009年3月采用自制问卷对其全体人员进行调查.结果 122名社区卫生服务工作人员中,有培训意愿者119人(占97.5%);111人次(91.0%)选择半脱产培训,在合理的培训时间选择中,98人次(80.3%)选择5～9月份.社区卫生工作人员认为需增加学时的培训内容集中在社区康复、社区急救、社区妇幼保健、慢性病防治、社区诊断和健康教育几个方面.培训中存在的主要问题包括培训内容太多、授课速度太快;培训工作与社区工作在时间上有冲突;理论与实践脱节;实习环节安排不合理等.结论要进一步提高认识,加强领导,推进全科岗位培训工作深入开展;强调"常见、多发"的原则下,按各培训对象的实际需要设计培训内容和形式;合理确定培训时间和环节,以促进宁夏全科医学培训工作的发展.

Objective To explore the problems of general medicine training in Ningxia and then to put out countermeasures for them. Methods A two-stage method of investigation was applied. The first was, from August to October 2008, interviewing with the relevant leaders from general medical training center of Ningxia, health department and the community health service institutions. The second was, from November 2008 to March 2009, using self-extracted questionnaires for the staffs from randomly selected 20 community health service stations and 4 community health service centers of five cities in Ningxia by using stratified cluster sampling. Results Among the 122 members of the staff 119 (97.5%) had a desire for training, 111 person-times (91.0%) chose the mode of partial-time training. Some 98 (80.3%) chose the time from May to September for their training. They held that the training contents need additional hours should be focused on community-based rehabilitation, first aid, maternal and child health care, chronic disease prevention and treatment, community-based diagnosis and health education. The main problems on training included too many contents with speedy lecture; time conflict between the training and community work; separation between theory and practice; and unreasonable internship. Conclusion To promote the development of general medicine training in Ningxia the recognition needs to be raised and the leadership to be enhanced. The training contents and modes should be adjusted according to the real demands of the training object, with a principle that the common and frequently-encountered diseases are emphasized. And the training time and link should be reasonably arranged.
