目的 检测Toll样受体2( toll-like receptor 2,TLR2)和Toll样受体4(toll-like receptor 4,TLR4)在慢性化脓性中耳炎、中耳胆脂瘤中的表达,探讨其在慢性化脓性中耳炎和中耳胆脂瘤发病中的作用.方法 选取耳硬化症患者(对照组)、慢性化脓性中耳炎患者(化脓性中耳炎组)、中耳胆脂瘤患者(中耳胆脂瘤组)各30例,应用实时定量聚合酶链反应( real-time PCR)、蛋白质印迹法( Western blot)、免疫组化检测TL R2/TLR4在正常外耳道皮肤,慢性化脓性中耳炎黏膜、肉芽组织,中耳胆脂瘤患者黏膜、肉芽组织及胆脂瘤囊壁中的表达,并比较表达程度的差异.结果 ①TLR2/TLR4mRNA及蛋白质在正常外耳道皮肤,慢性化脓性中耳炎中耳黏膜、肉芽组织,胆脂瘤的中耳黏膜、肉芽组织及胆脂瘤囊壁均有表达.②TLR2/TLR4 mRNA及其蛋白质在慢性化脓性中耳炎及胆脂瘤黏膜中的表达均高于正常外耳道皮肤(P值均<0.05),在胆脂瘤囊壁中的表达低于正常外耳道皮肤(P值均<0.05),两组黏膜中TLR2/TLR4的表达差异无统计学意义(P值均>0.05).③TLR2/TLR4mRNA及蛋白质在两组中耳炎肉芽组织中的表达均高于正常外耳道皮肤(P值均<0.01),且TLR2mRNA在中耳胆脂瘤肉芽组织中的表达高于慢性化脓性中耳炎(P<0.05).④TLR2/TLR4阳性细胞主要分布在肉芽组织中,且明显多于正常外耳道皮肤,但胆脂瘤囊壁中的阳性细胞少于正常外耳道皮肤.结论 TLR2和TLR4在正常外耳道皮肤、慢性化脓中耳炎及中耳胆脂瘤中均有表达,提示中耳具有TLR2、TLR4参与调节的固有免疫系统,但二者的差异性表达也提示其在慢性化脓性中耳炎和中耳胆脂瘤的发病中所起的作用不同.
目的 檢測Toll樣受體2( toll-like receptor 2,TLR2)和Toll樣受體4(toll-like receptor 4,TLR4)在慢性化膿性中耳炎、中耳膽脂瘤中的錶達,探討其在慢性化膿性中耳炎和中耳膽脂瘤髮病中的作用.方法 選取耳硬化癥患者(對照組)、慢性化膿性中耳炎患者(化膿性中耳炎組)、中耳膽脂瘤患者(中耳膽脂瘤組)各30例,應用實時定量聚閤酶鏈反應( real-time PCR)、蛋白質印跡法( Western blot)、免疫組化檢測TL R2/TLR4在正常外耳道皮膚,慢性化膿性中耳炎黏膜、肉芽組織,中耳膽脂瘤患者黏膜、肉芽組織及膽脂瘤囊壁中的錶達,併比較錶達程度的差異.結果 ①TLR2/TLR4mRNA及蛋白質在正常外耳道皮膚,慢性化膿性中耳炎中耳黏膜、肉芽組織,膽脂瘤的中耳黏膜、肉芽組織及膽脂瘤囊壁均有錶達.②TLR2/TLR4 mRNA及其蛋白質在慢性化膿性中耳炎及膽脂瘤黏膜中的錶達均高于正常外耳道皮膚(P值均<0.05),在膽脂瘤囊壁中的錶達低于正常外耳道皮膚(P值均<0.05),兩組黏膜中TLR2/TLR4的錶達差異無統計學意義(P值均>0.05).③TLR2/TLR4mRNA及蛋白質在兩組中耳炎肉芽組織中的錶達均高于正常外耳道皮膚(P值均<0.01),且TLR2mRNA在中耳膽脂瘤肉芽組織中的錶達高于慢性化膿性中耳炎(P<0.05).④TLR2/TLR4暘性細胞主要分佈在肉芽組織中,且明顯多于正常外耳道皮膚,但膽脂瘤囊壁中的暘性細胞少于正常外耳道皮膚.結論 TLR2和TLR4在正常外耳道皮膚、慢性化膿中耳炎及中耳膽脂瘤中均有錶達,提示中耳具有TLR2、TLR4參與調節的固有免疫繫統,但二者的差異性錶達也提示其在慢性化膿性中耳炎和中耳膽脂瘤的髮病中所起的作用不同.
목적 검측Toll양수체2( toll-like receptor 2,TLR2)화Toll양수체4(toll-like receptor 4,TLR4)재만성화농성중이염、중이담지류중적표체,탐토기재만성화농성중이염화중이담지류발병중적작용.방법 선취이경화증환자(대조조)、만성화농성중이염환자(화농성중이염조)、중이담지류환자(중이담지류조)각30례,응용실시정량취합매련반응( real-time PCR)、단백질인적법( Western blot)、면역조화검측TL R2/TLR4재정상외이도피부,만성화농성중이염점막、육아조직,중이담지류환자점막、육아조직급담지류낭벽중적표체,병비교표체정도적차이.결과 ①TLR2/TLR4mRNA급단백질재정상외이도피부,만성화농성중이염중이점막、육아조직,담지류적중이점막、육아조직급담지류낭벽균유표체.②TLR2/TLR4 mRNA급기단백질재만성화농성중이염급담지류점막중적표체균고우정상외이도피부(P치균<0.05),재담지류낭벽중적표체저우정상외이도피부(P치균<0.05),량조점막중TLR2/TLR4적표체차이무통계학의의(P치균>0.05).③TLR2/TLR4mRNA급단백질재량조중이염육아조직중적표체균고우정상외이도피부(P치균<0.01),차TLR2mRNA재중이담지류육아조직중적표체고우만성화농성중이염(P<0.05).④TLR2/TLR4양성세포주요분포재육아조직중,차명현다우정상외이도피부,단담지류낭벽중적양성세포소우정상외이도피부.결론 TLR2화TLR4재정상외이도피부、만성화농중이염급중이담지류중균유표체,제시중이구유TLR2、TLR4삼여조절적고유면역계통,단이자적차이성표체야제시기재만성화농성중이염화중이담지류적발병중소기적작용불동.
Objective To investigate the differential expression of toll-like receptor 2 ( TLR2 ),toll-like receptor 4 (TLR4) and their potential role in the pathogenesis of chronic suppurative otitis meida and cholesteatoma.Methods Normal cannal skin of 30 patients with tympanosclerosis were enrolled as control,30 cases with choronic suppurative otitis media and 30 patients with cholesteatoma were studied.Real-time PCR,Western blot and Immunohistochemistry were preformed to detect the expression of TLR2/TLR4 in normal canal skin,mucosa and granulation tissue of choronic suppurative otitis media,mucosa,granulation tissue,cholesteatoma epithelium of cholesteatoma,and the differential expression were ananlyzed.Results ①the mRNA and protein expression of TLR2 and TLR4 were detected in all normal canal skin,mucosa and granulation tissue of choronic suppurative otitis media,mucosa,granulation tissue,cholesteatoma epithelium of cholesteatoma.②Both mRNA and protein level of TLR2/TLR4 in mucosa of chronic suppurative otitis media and cholesteatoma were higher than those in normal canal skin,but lower in cholesteatoma epithelium,there was no significant difference in mucosa of the two otitis media groups.③The mRNA and protein expressiom of TLR2/TLR4 in granulation tissue of chronic suppurative otitis media and cholesteatoma were significant increased when compared with normal canal skin,and TLR2 expression level was higher in granulation tissue of cholesteatoma than in choronic suppurative otitis meida.④TLR2/TLR4 positive cells mainly infiltrated in granulations,significantly more than in normal skin,while fewer in the epithelium of cholesteatoma.Conclusions Differential expression of TLR2 and TLR4 in mucosa suggests middle ear is a TLR2/TLR4 participated functional modulation of the innate immune system and also suggests that they may play a different role in the pathophvsiology of chronic otitis media and cholesteatoma.
