目的 观察高压氧(HBO)治疗对内毒索诱导兔非显性弥散性血管内凝血(non-overt DIC)指标的影响.方法 48只新西兰兔按数字表法随机分为4组,每组12只.第1、2组耳缘静脉注射0.9%生理盐水,第1组为对照组,第2组为HBO对照组;第3.4组由耳缘静脉注射内毒素100 μg/kg制作non-overt DIC模型,第3组为non-overt DIC组,第4组为non-oven DIC+HBO组.第2、4组给予HBO处理,2次/d,共6次.每组在注射生理盐水或内毒素前和注射后6、12、24、48、72 h抽取耳中央动脉血,检测血小板、凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)、纤维蛋白原(FIB)、D-二聚体等DIC凝血指标.结果 Non-overt DIC组血小板、FIB与对照组和HBO对照组比较,相应时间点均有显著降低(P<0.05或P<0.01);non-overt DIC+HBO组较non-overt DIC组降低幅度小,但2组高峰期差异有统计学意义(P<0.05或P<0.01).non-overt DIC组PT、APTT、D-二聚体与对照组和HBO对照组比较,相应时间点均有显著升高(P<0.05或P<0.01);non-overt DIC+HBO组较non-overt DIC组上升幅度小,2组高峰期差异有统计学意义(P<0.05或P<0.01).结论 HBO可促进non-overt DIC兔的异常凝血指标恢复.

Objective To observe effects of hyperbaric oxygen (HBO) therapy on the indices of non-overt disseminated intravascular coagulation (DIC) induced by endotoxin in rabbits.Methods Forty-eight New Zealand rabbits were randomly divided into 4 groups,each consisting of 12.The first group (the control group) and the second group (the HBO control group) were given 10 ml NaCl with a concentration of 0.9%through ear edge vein.The third and the fourth group were administered endotoxin (100 μg/kg),through ear edge vein to induce non-overt DIC model.The 4th group was non-overt DIC+HBO group.The second and the fourth groups were given HBO treatment,twice a day,totaling 6 times.Blood samples were taken from ear central artery of each group either before injection of 0.9% NaCl/endotoxin,or after injection at hour 6,12,24,48,72 to detect indices of disseminated intravascular coagulation (DIC):platelet,prothrombin time (PT),activated partial prothrombin time (APTT),fibrinogen (FIB) and D-dimer.Results Platelet and FIB in the non-overt DIC group at corresponding time points were significantly lower than those of the control group and the HBO control group(P<0.05 or P<0.01),while the same indices of the non-overt DIC+HBO group dropped less significantly than those of the non-overt DIC group.However,statistical significance could be seen in cresttime.when a comparison was made between the two groups (P<0.05 or P<0.01).PT,APIT,D-dimer in the non-overr DIC group at corresponding time points all increased significantly than the control group and the HBO group(P<0.05 or P<0.01),while the same indices of the non-overt DIC+HBO group increased less significantiv than those of the non-overt DIC group,with statistical significance in crest-time,when a comparison was made between the two groups (P<0.05 or P<0.01).Conclusions HBO could effectively enhance recovery of platelets,PT,APTT,FIB,D-dimer in rabbits.
