本文报道采用Mollard(1978)的前会阴切口径路手术治疗7例高、中位无肛患儿,术后均获得满意疗效,无大便失禁.有2例术后污粪系粘膜外翻所致,再次作粘膜切除后污粪现象消失.本术优点在于解剖清晰,手术易行,能直视尿道球部、耻骨直肠肌环和外括约肌,并不易损伤这些结构.

From Nov. 1985 to Sept. 1988,7 Patients were treated for high or intermediate imperforate anus with anterior perioneal approach described by Mollard(1978). After operation two cases complicated with aversion of anal mucosa,the others got satisfactory outcome. None including one with fecal incontienceresulted from parevious operation in another hospital had fecal incontinence. Based on Pe(n)a's opinion(1982)that the external sphincter muscle is a functionally useful structure,we have adopted a modified wing type perineal incision for exposing the external sphincter and reconstructing the anus. This approach is easy and less dumaging,and provides satisfactory exposure of important anatomical and functional structures such as bulbo-urethra,puborectalis sling and external sphincter muscles.
