目的:探讨低钙透析液联合鲑鱼降钙素对维持血透(Maintenance hemodialysis, MHD)患者高磷血症的影响。方法选取我院近期维持性血液透析患者中高磷血症患者40例,采用随机数字表法分为对照组21例,给予常规钙透析液+活性维生素D3治疗;试验组19例,给予低钙透析液联合鲑鱼降钙素注射液治疗。观察两组患者治疗后第3、6个月时血清钙磷代谢等指标有无差别。结果对照组治疗后血清钙较治疗前无明显变化(P>0.05),但血磷和甲状旁腺激素(iPTH)较透析前明显增加(P<0.05);试验组治疗后血清钙、iPTH无明显变化(P>0.05),血磷显著降低(P<0.05)。结论鲑鱼降钙素联合低钙透析液治疗MHD能有效降低患者血磷水平且不致影响iPTH。

목적:탐토저개투석액연합해어강개소대유지혈투(Maintenance hemodialysis, MHD)환자고린혈증적영향。방법선취아원근기유지성혈액투석환자중고린혈증환자40례,채용수궤수자표법분위대조조21례,급여상규개투석액+활성유생소D3치료;시험조19례,급여저개투석액연합해어강개소주사액치료。관찰량조환자치료후제3、6개월시혈청개린대사등지표유무차별。결과대조조치료후혈청개교치료전무명현변화(P>0.05),단혈린화갑상방선격소(iPTH)교투석전명현증가(P<0.05);시험조치료후혈청개、iPTH무명현변화(P>0.05),혈린현저강저(P<0.05)。결론해어강개소연합저개투석액치료MHD능유효강저환자혈린수평차불치영향iPTH。
Objective To evaluate the effect of low calcium dialysate combined with salmon calcitonin in the treatment of hyper-phosphatemia in hemodialysis patients.Methods Forty hemodialysis patients with hyperphosphaemia were randomly divided into the treatment group (n=19) and the control group (n=21).Patients in the control group were treated with low calcium dialysate (with Ca+ concentration of 1.5 mmo1 )combined with calcium carbonate as well as vitamin D3 ,while those in the treatment group were treated with low calcium dialysate (with Ca2+ concentration of 1.25 mmo1)combined with salmon calcitonin,as well as calcium carbonate and vitamin D3.The Ca,phosphate product and iPTH were detected three and six months after treatment.Results In the control group, the serum phosphate and iPTH was significantly increased after treatment ( P<0.05); But in the treatment group, the serum phosphate level was significantly decreased(P<0.05) without changes of serum calcium and iPTH(P>0.05). Conclusion For hemodialysis patients with hyperphosphatemia ,low calcium dialysate combined with salmon calcitonin can effectively reduce serum phosphate level with no effect on iPTH.
