目的 观察p38 MAPK抑制剂SB203580对兔滤过手术后纤维化反应的作用.方法 将12只兔24只眼随机分为3组:A组(单纯滤过手术组)、B组(滤过手术+SB203580组)、C组(滤过手术+MMC组).对各组进行裂隙灯显微镜观察、眼压检查、滤过区结膜组织学和透射电镜观察、免疫组织化学法检查滤过区α-SMA表达.结果 术后14dA组滤过泡血管化、瘢痕形成,B组滤过泡扁平弥散,C组滤过泡苍白缺血状、扁平弥散.不同时间点各组之间眼压无明显差异.滤过泡组织学及透射电镜观察检查可见不同组间结膜上皮细胞、成纤维细胞和上皮下纤维组织形态不同.免疫组织化学法观察见A组纤维组织间大量的棕黄色α-SMA阳性细胞,B、C组阳性细胞数量减少.结论 p38 MAPK抑制剂SB203580能减轻兔滤过手术后纤维化反应,抑制p38 MAPK信号通道可作为新的抗瘢痕化途径.

Objective To investigate the fibrosis suppression after trabeculectomy in rabbits by a p38 MAPK chemical inhibitor SB203580. Methods Twenty-four eyes of 12 rabbits were randomly divided into 3 groups:group A (trabeculectomy alone group),group B (trabeculectomy + SB203580group),group C (trabeculectomy + MMC group).Bleb appearance and intraocular pressure were observed during a 14-d period.Histological and transmission electron microscopy examinations,immunohistochemistry observations were evaluated. Results SB203580 enhanced the surgical outcome and inhibited fibroblast differentiation when compared with PBS.Bleb appearance in the SB203580-treated eyes were much diffuse and less neovascularization than those in the trabeculectomy alone eyes on d7 and d14.No significant differences in IOPs were observed between groups.Histological and ultrastructure analysis showed that conjunctival epithelium cells,fibroblasts and fibrous tissue differed between groups. Conclusions Fibrotic reaction after trabeculectomy in rabbits is suppressed by inhibitor p38 MAPK signal pathway.
