目的 探讨地拉罗司能否通过诱导上皮细胞发生间质化改变,从而促进皮瓣微血管生成.方法 雄性成年SD大鼠32只随机均分为Ⅰ组和Ⅱ组,2组又分别分为a、b组,每组各8只.Ⅰa组连续7d灌胃地拉罗司100 mg/kg,Ⅰb组连续7d灌胃等量生理盐水,灌胃结束后,次日行大鼠背部窄蒂皮瓣成形术,术中观察皮下微血管情况,术后第7天观察皮瓣愈合情况,并计算皮瓣成活面积百分比.Ⅱa组连续7d灌胃地拉罗司100 mg/kg,Ⅱb组连续7d灌胃等量生理盐水,灌胃结束后,次日行大鼠背部窄蒂皮瓣成形术,术后第3天皮瓣远端采集标本,免疫组化和Western blot法检测分化簇因子34(C D34)、E钙黏素、波形蛋白的含量,并计算皮瓣微血管密度.结果 Ⅰa组皮瓣下微血管生长较Ⅰb组旺盛.Ⅰa组皮瓣存活率100%,Ⅰb组存活率62.5% (P <0.05);Ⅰa和Ⅰb皮瓣成活面积百分比分别为(100±0.OO)%和(84.06±4.42)%(P<0.05).免疫组化及Western blot法检测示lⅡa组E钙黏素低于Ⅱb,而波形蛋白、CD34的表达量高于Ⅱb(P<0.05).结论 地拉罗司能够通过诱导上皮细胞发生间质化改变促进皮瓣组织微血管生成,具有较强的促进窄蒂皮瓣愈合的作用.

목적 탐토지랍라사능부통과유도상피세포발생간질화개변,종이촉진피판미혈관생성.방법 웅성성년SD대서32지수궤균분위Ⅰ조화Ⅱ조,2조우분별분위a、b조,매조각8지.Ⅰa조련속7d관위지랍라사100 mg/kg,Ⅰb조련속7d관위등량생리염수,관위결속후,차일행대서배부착체피판성형술,술중관찰피하미혈관정황,술후제7천관찰피판유합정황,병계산피판성활면적백분비.Ⅱa조련속7d관위지랍라사100 mg/kg,Ⅱb조련속7d관위등량생리염수,관위결속후,차일행대서배부착체피판성형술,술후제3천피판원단채집표본,면역조화화Western blot법검측분화족인자34(C D34)、E개점소、파형단백적함량,병계산피판미혈관밀도.결과 Ⅰa조피판하미혈관생장교Ⅰb조왕성.Ⅰa조피판존활솔100%,Ⅰb조존활솔62.5% (P <0.05);Ⅰa화Ⅰb피판성활면적백분비분별위(100±0.OO)%화(84.06±4.42)%(P<0.05).면역조화급Western blot법검측시lⅡa조E개점소저우Ⅱb,이파형단백、CD34적표체량고우Ⅱb(P<0.05).결론 지랍라사능구통과유도상피세포발생간질화개변촉진피판조직미혈관생성,구유교강적촉진착체피판유합적작용.
Objective To investigate the effect of Deferasirox on the micro-angiogenesis in narrow pedicle flap through Epithelial-Mesenchymal Transition.Methods 32 male rats were randomly divided into group Ⅰ and Ⅱ which were subdivided into Ⅰa and Ⅰb,Ⅱa and Ⅱb,8 rats in each group.The rats were administrated intragastrically for 7 days with Deferasirox 100 mg/kg in group Ⅰa and Ⅱa,with the same dose of N.S.in group Ⅰb and Ⅱb.After that,narrow pedicle flaps were formed on the rats back.In group Ⅰ,the subcutaneous vascular network was observed intraoperatively. The flap survival rate was recorded.In group Ⅱ,specimens were collected at the distal end of flaps 3 days after operation.IHC and Western Blot were done to examine the expression of CD34 、E-cadherin、Vimentin.The microvessel density was also calculated.Results The subcutaneous micro-angiogenesis in group Ⅰa was more exuberant than that in group Ⅰb.The narrow pedical flaps in group Ⅰa survived completely,while the survival rate was62.5% in group Ⅰb ( P < 0.05 ).The percentage of flap survival area for Ⅰa and Ⅰb was ( 100 ± 0.00) %and (84.06 ± 4.42) % ( P < 0.05).The expression of E-cadherin in Ⅱa was lower than that in Ⅱb,whilethe expression of Vimentin and CD34 were higher in Ⅱa,showing statistically difference ( P < 0.05).Conclusion Deferasirox can improve the flap micro-angiogenesis through inducing epithelialmesenchymal transition,so as to improve the survival rate of narrow pedicle flap.
