目的 观察Zn2+改性羧甲基纤维素( Zn2+ -SCMC)对术后腹腔粘连的预防作用.方法 将90只大鼠随机分为A、B、C3组,每组各30只.先行均制作肠粘连模型,取A组腹腔内不放药物,B、C组于腹腔内损伤部位分别置入3ml、浓度3%的羧甲基纤维素(SCMC)及3ml、浓度3%的Zn2+ -SCMC,术后10d处死大鼠,观察各组腹腔粘连情况.结果 大鼠肠壁组织病理:A组纤维细胞增生明显,胶原纤维排列致密;B组纤维细胞增生较明显,胶原纤维排列略密;C组纤维细胞增生较轻,胶原纤维排列较疏松.术后10d腹腔内粘连情况:C组腹腔内轻微粘连,粘连发生率44.8%,B组中度粘连,粘连发生率80.0%,A组广泛粘连,致密,粘连发生率100.0%.A组Ⅲ~Ⅳ级粘连最多为17只,B组Ⅱ级粘连最多12只,C组0级粘连最多16只,C组粘连分级较B组、A组明显减轻,差异有统计学意义(P<0.01).结论 Zn2+ -SCMC能明显减轻术后腹腔粘连程度,其作用优于SCMC.

목적 관찰Zn2+개성최갑기섬유소( Zn2+ -SCMC)대술후복강점련적예방작용.방법 장90지대서수궤분위A、B、C3조,매조각30지.선행균제작장점련모형,취A조복강내불방약물,B、C조우복강내손상부위분별치입3ml、농도3%적최갑기섬유소(SCMC)급3ml、농도3%적Zn2+ -SCMC,술후10d처사대서,관찰각조복강점련정황.결과 대서장벽조직병리:A조섬유세포증생명현,효원섬유배렬치밀;B조섬유세포증생교명현,효원섬유배렬략밀;C조섬유세포증생교경,효원섬유배렬교소송.술후10d복강내점련정황:C조복강내경미점련,점련발생솔44.8%,B조중도점련,점련발생솔80.0%,A조엄범점련,치밀,점련발생솔100.0%.A조Ⅲ~Ⅳ급점련최다위17지,B조Ⅱ급점련최다12지,C조0급점련최다16지,C조점련분급교B조、A조명현감경,차이유통계학의의(P<0.01).결론 Zn2+ -SCMC능명현감경술후복강점련정도,기작용우우SCMC.
Objective To investigate the Zn2+ modifical sodium carboxymethyl cellulose (Zn2+-SCMC) on the prevention of postoperative abdominal adhesion.Methods Ninety rats were randomly divided into three groups:group A,group B and group C.They were first produced adhesions model.Then group A of drugs hold the abdominal cavity,but group B,group C were injected intra-abdominal at injured site 3% sodium carboxymethyl cellulose (SCMC) 3 ml,and 3% Zn2+ -SCMC 3 ml separately.Each group was sacrificed on and abdominal adhesions were observed.Results Pathological change:group A was significantly more intestinal fibroblast proliferation,collagen fibers arranged in dense,group B was slight more intestinal fibroblast proliferation,collagen fibers arranged in slight dense ;group C were less intestinal fibroblast proliferation,collagen fibers Arranged in loose.The circumstance of abdominal adhesions after postoperative 10 days:Group C minor intra-abdominal adhesions,the rate of abdominal adhesions is 44.8%.Group B with moderate adhesion,the rate of abdominal adhesions is 80.0%.But group A extensive adhesions and dense,the rate of abdominal adhesions is 100.0%.There are at most 17 rats on level Ⅲ-Ⅳadhesions of group A.There are at most 12 rats on level Ⅱ adhesions of group B.And there are at most 16 rats on level 0 adhesions of group C.The adhesions grade of Group C obviously lighten than another two group.There is significant difference in adhesion grade ( P < 0.0l ).Conclusion Zn2+ -SCMC significantly reduce postoperative abdominal adhesions,its role is better than the SCMC.
