目的 探讨CRH、CORT、IL-1、6、8在足月分娩发动及分娩过程中的作用.方法 选择2005年1月至2006年6月福建省妇幼保健院住院的单胎足月妊娠初产妇37例,动态测定临产前后其血清CRH、CORT、IL-1、6、8水平,进行对比分析,样本分为3组:A组临产前组、B组潜伏期组、C组活跃期组(分为C1组活跃期进展顺利最后经阴道分娩者23例及C2组活跃期进展异常最后经剖宫产分娩者14例);血清CRH、CORT、IL-1、6、8浓度均采用ELISA法测定.结果 ①B组、C组血CRH分别为(111.28±24.16)nmol/L、(122.22±28.85)nmol/L较A组(76.05±22.03)nmol/L明显升高(P<0.001),B组与C组比较无统计学意义(P>O.05);A组血CORT、IL-1、6、8水平分别为(323.71±146.24)ng/L、(95.34±26.88)nmol/L、(5.17±2.37)pg/ml、(56.46±18.37)pg/ml;B组血CORT、IL-1、6、8水平分别为(459.48±209.84)ng/L、(131.61±27.39)nmol/L、(12.60±3.55)pg/ml、(134.07±39.86)pg/ml,C组血CORT、IL-1、6、8水平分别为(577.29±173.55)ng/L、(171.06±56.21)nmol/L、(21.82±9.45)pg/ml、(193.07±64.90)pg/ml,B、C组各因子水平均高于A组,且C组高于B组,差异均有统计学意义(均P<0.05);②C1组血CORT、IL-1、6、8分别为(629.28±191.42)ng/L、(186.24±64.30)nmol/L、(24.58±11.31)pg/ml、(212.12±71.92)pg/ml,C2组血CORT、IL-1、6、8水平分别为(491.87±93.59)ng/L、(146.12±26.01)nmol/L、(17.58±8.66)pg/ml、(161.77±34.83)pg/ml,C1组比C2组升高,差异有统计学意义(均P<0.05);③潜伏期足月孕妇血清CORT、IL-1、6、8水平与潜伏期时间呈负相关,且CORT与IL-1、6、8水平呈正相关;④活跃期足月孕妇血清C0RT、IL-1、6、8水平与活跃期时间呈负相关,且CORT与IL-1、6、8水平呈正相关.结论 CRH可能是分娩发动的始动因子;CORT、IL-1、6、8可能是分娩发动及加速分娩的重要因子;产程中联合监测CORT、IL-1、6、8水平动态变化有助于判断产程进展情况.

Objective The purpose of the study was to explore the relationship between corticotropinreleasing hormone(CRH) ,cortisoil(CORT) ,Interleukin(IL)-1 ,IL-6,IL-8 and term parturition. Methods Serum CRH,CORT, IL-1,6,8 levels were monitoring measured by enzyme-linked immunosorbent assay from 37 term pregnancy women. Serum samples were divided into three groups. Group A before labor;Group B in latent phase; Group C in active phase ,which divided into C1 sub-group viginal diliver( n = 23 ) and C2 sub-group cesarean section ( n = 14). Results ①CRH was significantly lower in group A (76. 05 ± 22. 03 ) nmol/L than in group B ( 111.28 ± 24. 16 ) nmol/L and in group C ( 122. 22 ± 28.85 ) nmol/L ( P < 0. 001 ) , but there was no significant difference in group B and group C ( P > 0. 05 ); The serum levels of CORT, IL-1,6,8 in group A were ( 323.71 ± 146. 24 ) ng/L, ( 95.34 ± 26. 88 ) nmol/L, ( 5.17 ± 2. 37 ) pg/ml, ( 56. 46 ± 18.37 ) pg/ml respectively ;in group B were (459.48 ± 209. 84 ) ng/L, ( 131.61 ± 27.39 ) nmol/L, ( 12. 60 ± 3.55 ) pg/ml, ( 134. 07± 39. 86) pg/ml respectively;in group C were(577.29 ± 173.55 ) ng/L、 ( 171.06 ± 56. 21 ) nmol/L、 ( 21.82 ±9. 45) pg/ml、( 193.07 ± 64. 90) pg/ml respectively. There was significant difference among CORT, IL-1、6、8 levels in group A, group B, group C (P < 0. 05 ). ②The serum levels of CORT、IL-1、6、8 in group C1 were ( 3629. 28 ± 191.42 ) ng/L、 ( 186. 24 ± 64. 30 ) nmol/L、 ( 24. 58 ± 11.31 ) pg/ml、 ( 212. 12 ± 71.92 ) pg/ml respectively, which were significantly higher than in group C2 ( 491.87 ± 93.59 ) ng/L、 ( 146. 12 ± 26. 01 ) nmol/L、 (17.58 ± 8.66)pg/ml、 (161.77 ± 34. 83 )pg/ml;③The serum CORT, IL-1,6,8 levels in group B were negatively related to the length of the latent phase. The serum CORT level was positively related to the serum IL-1,6,8 levels;④The serum CORT,IL-1,6,8 levels in group C were negatively related to the length of the active phase. The serum CORT level was positively related to the serum IL-1、6、8 levels. Conclusion The CRH may play imporant role in the onset of term 1abour,the CORT,IL-1、6、8 may play imporant role in the onset of term 1abour and the acceleration of 1abour; Measurement of those cytokines may be useful in judging the delivery process.
