目的 探讨初诊2型糖尿病(T2DM)合并高血压患者血浆Chemerin水平变化及其与血压、WHR、血糖、血脂、血浆胰岛素水平等的关系.方法 酶联免疫法测定了初诊T2DM患者(T2DM组)、T2DM合并高血压患者(T2DM+HT组)和正常人(NGT组)血浆Chemerin水平,分析其与血压、BMI、WHR、血脂、HOMA-IR等的关系.结果 T2DM+HT组血浆Chemerin水平明显高于T2DM组和NGT组.血浆Chemerin水平与年龄、体重、腰围、臀围、DBP、体内脂肪百分比(FAT%)、BMI、WHR、TG、FFA、FPG、2hPG、FIns、2hPIns、HOMA-IR、HbA1c呈正相关.多元回归分析结果表明腰围、DBP、2hPIns、HbA1c是影响血浆Chemerin水平的独立相关因素.结论 血浆Chemerin水平的改变与糖脂代谢和血压等有关,并可能参与了胰岛素抵抗及糖尿病合并高血压的发生发展.

ObjectiveTo investigate the changes of plasma Chemerin level and the relationship between plasma Chemerin level and blood pressure, waist hip ratio (WHR), blood lipids, blood glucose, plasma insulin levels as well as other factors in newly diagnosed type 2 diabetic patients with hypertension.MethodsPlasma Chemerin levels were assayed by ELISA in the newly diagnosed type 2 diabetes mellitus (T2DM) , type 2 diabetic patients with hypertension (T2DM+HT) and normal controls (NC). The relationship between plasma Chemerin levels and blood pressure, BMI, WHR, blood lipids and HOMA-insulin resistance index (HOMA-IR ) as also analyzed.ResultsPlasma Chemerin level in T2DM+HT was significantly increased,as compared with T2DM and NC [(74.6±6.4) vs (68.6±12.5 )and (62.1±19.3)μg/L,both P<0.01]. Plasma Chemerin level correlated positively with age, body weight, waist circumference(WC), hip circumference, DBP,visceral fat%(FAT%), BMI, WHR, TG, FFA, FPG, 2hPG, FIns, 2hPIns, HOMA-IR, HbA1c(r=0.20, 0.25, 0.33, 0.20, 0.27, 0.30, 0.34, 0.24, 0.25, 0.16, 0.17, 0.21, 0.18, 0.21, 0.21, 0.19 ,respectively P<0.01 and P<0.05).Multiple regression analysis showed that WC, DBP, 2hPINS, HbA1c were independent related factors influencing plasma Chemerin level.ConclusionsThe changes of Chemerin levels are associated with glucose and lipid metabolism and blood pressure, and it may contribute, in part, to the pathogenesis of IR and diabetes complicated with hypertension.
