目的 调查表面健康人群血清氨基末端B型钠尿肽原(NT-proBNP)的参考区间.方法 参照美国临床实验室标准化委员会(NCCLS)C28-A2文件对德灵公司Dimension X-PAND生化分析仪及NT-proBNP原装配套试剂(简称X-PAND NT-proBNP检测系统)的性能进行验证和评价.检测261名表面健康人群(男127名、女134名)的NT-proBNP水平,通过Z检验,按照性别分组,以第95百分位数分别建立男、女的参考区间.结果 X-PAND NT-proBNP检测系统性能良好,符合临床要求.NT-proBNP随着年龄的增长呈上升趋势,且女性的水平明显高于男性.检测结果 呈偏态分布,<75岁男性的参考区间为<64.4 ng/L,<75岁女性的参考区间为<122.4 ng/L.结论 对于存在明显种族差异、生物变异的检测项目,每个实验室应参照NCCLS C28-A2对参考区间进行调查或采取纠正措施.建立合适的NT-proBNP参考区间对心力衰竭的诊断、鉴别诊断及预后分析具有临床应用价值.

Objective To investigate the distribution of reference intervals of serum N-terminal pro-B-type natriuretic peptide (NT-proBNP). Methods According to the National Committee for Clinical Laboratory Standards(NCCLS)C28-A2 Document, the performance of X-PAND NT-proBNP determination system (including the Dade Behring Dimension X-PAND biochemistry analyzer and NT-proBNP original reagent) was analyzed and evaluated. The serum NT-proBNP levels in 261 apparently healthy individuals (127 males, 134 females) were determined. All individuals were classified according to sex. The NT-proBNP reference intervals of males and females were established by 95~(th) percentile respectively. Results The X-PAND NT-proBNP determination system had good analytical performance. Following the increasing age, the NT-proBNP level also increased, and the level in females was higher than that in males. The results of serum NT-proBNP were presented as skewness distribution. The reference interval of NT-proBNP was less than 64.4 ng/L for males and less than 122.4 ng/L for females under the age of 75. Conclusions Clinical laboratories should investigate or establish the appropriate reference intervals for the tests items with ethnic distinction and biomutation. There is a clinical application value to establish the appropriate reference interval for serum NT-proBNP in the diagnosis, differential diagnosis and prognosis analysis of heart failure.
