目的总结儿童外伤性脾破裂非手术治疗的护理经验.方法明确伤情观察指标,保证绝对卧床休息,保持大便通畅,保证输液输血管道通畅,做好术前准备,做好儿童及家属心理护理及出院指导.结果本组非手术治疗17例(占51.5%),成功15例,非手术治疗的选择率为51.5%,成功率为88.2%.其中2例非手术治疗期间出现延迟性出血,中转手术,17例均治愈出院.结论在严格掌握适应证,护理工作的密切配合下,密切动态观察伤情变化和有中转手术条件,儿童外伤性脾破裂进行非手术治疗是安全可行的,可避免不必要的手术切脾,降低并发症,减轻患儿痛苦,降低住院费用.

Objective To summarize the nursing experience of nonoperative treatment on spleen rupture of children.Methods The traumatic condition and the observe index must be defined,and absolute bed rest,bowels open,smooth transfusion tubing,preoperative preparation,psychological nursing and discharge guidance must be ensured.Results Of the 17 cases received nonoperative treatment,2 were operated after the failure of nonoperative treatment due to the occurrence of delayed hemorrhage.The selective rate of nonoperative management was 51.5%,the successful ratio was 88.2%.17 cases were all cured.Conclusions The nonoperative treatment of spleen rupture in children was safe and available when indication was strictly controlled.It can avoid unnecessary splenectomy surgery and reduce the complicated disease,alleviate pain in children and reduce hospitalization costs.
