目的 研究弧菌显色培养基对副溶血性弧菌(Vp)的筛选性能及Vp在临床中的检出率和抗菌谱.方法 肠道病患者粪便样本经碱性蛋白胨水增菌后,接种于弧菌显色培养基上,对淡紫色菌落进行生化鉴定分析,得出弧菌显色培养基的选择性能;药敏实验采用K-B法.结果 802份粪便样本经碱性蛋白胨水增菌和弧菌显色培养基培养后,193份样本中长出淡紫色菌落.附加试验和生化反应符合Vp标准的菌株为178株,检出率为22.2%,弧菌显色培养基鉴定Vp的阳性预测值为92.0%.药敏结果显示,Vp对氨苄西林耐药率达82.0%,对Ⅰ代头孢菌素(头孢噻吩、头孢唑林)的耐药率为22.5%,而对Ⅲ代头孢菌素、氨基糖苷类、Ⅲ代喹诺酮类及其他抗生素的耐药率<5.0%.结论 弧菌显色培养基对肠道门诊患者Vp的检出简便快速,准确度较高;需要时可选用第Ⅲ代喹诺酮类或头孢菌素、氨基糖苷类和哌拉西林等抗生素进行治疗.

Objective To study the feasibility of vibrio chromogenic medium isolating Vibrio parahaemolyticus(Vp)and the clinical Vp isolation and resistance rate of antibiotics.Method After bacterial enrichment in alkaline peptone water,stool samples of patients were inoculated in vibrio colored medium,and Vp tradidonal biochemical tests were applied later to identify the purple colonies on the plate.The resistance to antibiotics was carried out by K-B test.Results In 802 samples of diarrhea-out patient,there were 193 samples growing purple colonies on vibrio colored medium,in which 178 strains were identified as Vp by traditional biochemical tests. The clinical isolating rate of Vp was 22.2%and the positive predictive value for Vp of the colored medium was 92.0%.The rates of resistance to ampicillin.cefalothin and cefazolin were 82.0%.22.5%and 22.5% respectively.The rates of resistance to other drugs used in this study were all below 5.0%.Conclusions The vibrio colored medium on isolating Vp is fast,simple and has good accuracy.The third generation of quinolones and cephalosporin,and the aminoglycosides could be chosen to treat Vp infection when needed.
