目的 探讨经颅多谱勒(TCD)对偏头痛患者间歇期诊断价值.方法 对86例偏头痛患者经TCD直接检测颅内主要动脉的血液动力学变化.结果 TCD结果显示86例患者中血液速度加快65例,血液速度减慢7例,血流速度正常14例中,存在两侧血流速度不对称者5例.伴有频谱形态改变者42例,血流音粗糙或高调者51例.对照组中血流速度加快5例,减慢者1例,频谱形态改变者2例,血流音粗糙3例,两组间TCD结果比较,有显著差异.结论 TCD能较好地反映偏头痛患者血液动力学变化,对偏头痛的临床诊断和有效治疗能提供较好的帮助,且检测无痛、无创伤能反复检查,值得临床推广.

Objective Search the value of transcortical doppler in diagnosing migraine. Methods Observe the change of 86 cases eneephalic primary artery blood dynamics by transcortical doppler in 86 eases migraine diapause. Results 65 eases Transcorticeal doppler presented blood stream speedup in 86 eases migraine,7 eases presented blood stream tardiness. There are 5 cases asymmetry of two sides blood stream in 14 cases normal blood stream. 42 eases presented of transcortical doppler configuration change. 51 cases presented the coarse or high tonality blood stream sound. 5 cases presented blood stream speedup,1 eases presented blood stream tardiness,2 eases presented of transcortical doppler configuration change,3 eases presented the coarse blood stream sound in control team. There is distinct difference between control team and experiment team. Conlusion Transcortical doppler can preferably reflect the blood stream dynamics change,so transcortical doppler is an effective method for diagnosing migraine,and is indolenee,non-invasive examination.
