目的 研究重症监护病房(intensive care unit,ICU)机械通气引发呼吸机相关性肺炎(ventilator associated pneumonia,VAP)院内感染病原体的构成及耐药情况.方法 对2006年1月至2007年12月我院ICU收治的96例机械通气引发的VAP患者的感染菌及其耐药情况进行回顾性调查分析.结果共检出感染菌161株,其中革兰阴性杆菌104株(64.6%),革兰阳性球菌32株(19.9%),真菌25株(15.5%),药敏结果 显示菌株多重耐药现象严重.结论 ICU机械通气VAP院内感染病原体以革兰阴性杆菌为主,药敏试验为多重耐药,l临床应重视ICU机械通气引发的院内感染,重视病原菌的培养鉴定,合理使用抗生素.

Objective To study the pathogen of ventilator associated pneumonia(VAP) tract infection in patients receiving mechanical ventilation in intensive care unit(ICU) and the drug resistance of pathogens. Methods The pathogens and their drug resistance of VAP in 96 patients (from Jan. 2006 to Dec. 2007) receiving mechanical ventilation in the ICU were investigated. Results 161 strains of pathogens were isolated by bacterial culture and identified. The ratio of the gram-negative bacteria to total bacteria isolated was 64.6% (104 strains) and that of the gram-positive bacteria and the fungi were 19.9% (32 strains) and 15.5% (25 strains) respectively. The results showed mtiltiple drug-resistance of the strains. Conclusions Gram-negative bacteria are the majority of the pathogens from the patients receiving mechanical ventilation of VAP tract infection in ICU. The pathogens show multiple drug-resistance in drug sensitive test. It is suggested that great attention should be paid to the infections in patients receiving mechanical ventilation in ICU along with the cultivation and surveilance of pathogen so as to use antimicrobial agents rationally during the clinical therapy.
