目的:探讨经闭孔尿道中段阴道无张力悬吊术(TVT-O)治疗女性压力性尿失禁的疗效及安全性.方法:2006年1月至2008年11月在本院妇产科收治的行TVT-O手术的压力性尿失禁患者,共105例,术前、术后行1小时尿垫试验,填写尿失禁生活质量问卷和性生活质量问卷并总结.结果:①单纯行TVT-O手术用时30.4±10.2(15～60)分钟,术中平均出血33.8±13.8(10～50)ml.术后并发症很少.②1小时尿垫试验显示术后漏尿量与术前相比显著减少(P<0.01),手术客观治愈率术后6周、3个月、6个月和1年分别为85.1%,86.2%,86.5%和84.0%.③尿失禁生活质量问卷评分术后6个月和1年与术前相比差异有高度统计学意义,P<0.01.④性生活质量问卷评分显示术后6个月和1年与术前相比,性生活质量有改善趋势,但差异无统计学意义(P>0.05).结论:TVT-O手术操作简便,并发症少,短期疗效满意,对患者生活质量有很大改善,对性生活的影响及其长期疗效还有待进一步研究.
목적:탐토경폐공뇨도중단음도무장력현조술(TVT-O)치료녀성압력성뇨실금적료효급안전성.방법:2006년1월지2008년11월재본원부산과수치적행TVT-O수술적압력성뇨실금환자,공105례,술전、술후행1소시뇨점시험,전사뇨실금생활질량문권화성생활질량문권병총결.결과:①단순행TVT-O수술용시30.4±10.2(15～60)분종,술중평균출혈33.8±13.8(10～50)ml.술후병발증흔소.②1소시뇨점시험현시술후루뇨량여술전상비현저감소(P<0.01),수술객관치유솔술후6주、3개월、6개월화1년분별위85.1%,86.2%,86.5%화84.0%.③뇨실금생활질량문권평분술후6개월화1년여술전상비차이유고도통계학의의,P<0.01.④성생활질량문권평분현시술후6개월화1년여술전상비,성생활질량유개선추세,단차이무통계학의의(P>0.05).결론:TVT-O수술조작간편,병발증소,단기료효만의,대환자생활질량유흔대개선,대성생활적영향급기장기료효환유대진일보연구.
Objective:The effectiveness and safety of tension-free vaginal tape-obturator technique(TVT-O) for femal stress urinary incontinence was assessed. Methods:105 patients in Peking University First Hospital from January,2006 to November,2008 who had TVT-O were recruited. The patients were followed up before and after operation by 1h pad test and incontinence quality of life questionnaire( I -QoL) and quali-ty of sexual life questionnaire (PISQ). Results:① The mean time of operation was 30.4 ± 10.2 minutes ( 15 ～ 60 min ), mean bleed loss was 33.8 ± 13.8 ml ( 10 ～ 50 ml). There were very few complications of the operation. ② The 1h pad test of 6 weeks,3 months,6 months and 1year after operation showed the leakage significantly decreased than those preoperatively( P < 0.01 ). The objective cure rates were 85.1% ,86.2%, 86.5% and 84% respectively. ③ The score of I -QoL was better postoperatively than preopratively( P < 0.01 ). ④The score of PISQ was no difference postoperatively (6 months and 1year) compared with preop-eratively(P>0.05). Concluslons:TVT-O is a surgery that is easy to operate. It has few complications, satis-fied short-term cure rate and quality of life. The impact on sexual life and long-term results still need further e-valuation.
