探讨右室起搏前后患者血管活性物质水平与心功能的相关性。采用放射免疫法测定37例右室起搏术前后和30例正常对照组血内皮素(ET)、心钠素(ANP) 、神经肽Y(NPY)、降钙素基因相关肽(CGRP)、神经降压素((NT)、一氧化氮(NO)水平，同时应用彩色多普勒超声心动图测定起搏治疗前后患者的左、右心功能及血液动力学参数。结果：与健康人相比，右室起搏术前后患者血ET、ANP、CGRP、NPY升高，NT、NO下降；手术治疗前后相比，无起搏综合征组的血ET、CGRP无明显变化，ANP、NPY明显下降，血NT、NO明显升高；起搏综合征组的血ET、ANP、CGRP、NPY、NO均明显升高，而NT明显下降，这种变化在第 4周比第1周更明显；且血ET、ANP、CGRP、NPY、NO与肺动脉平均压呈正相关，与左、右室射血分数呈正相关。结论：起搏综合征患者存在着明显的血管活性物质水平失衡。
탐토우실기박전후환자혈관활성물질수평여심공능적상관성 。채용방사면역법측정37례우실기박술전후화30례정상대조조혈내피소(ET)、심납소(ANP) 、신경태Y(NPY)、강개소기인상관태(CGRP)、신경강압소((NT)、일양화담(NO)수평，동시 응용채색다보륵초성심동도측정기박치료전후환자적좌、우심공능급혈액동역학삼수。결과 ：여건강인상비，우실기박술전후환자혈ET、ANP、CGRP、NPY승고，NT、NO하강；수술치료 전후상비，무기박종합정조적혈ET、CGRP무명현변화，ANP、NPY명현하강，혈NT、NO명현승 고；기박종합정조적혈ET、ANP、CGRP、NPY、NO균명현승고，이NT명현하강，저충변화재제 4주비제1주경명현；차혈ET、ANP、CGRP、NPY、NO여폐동맥평균압정정상관，여좌、우실사 혈분수정정상관。결론：기박종합정환자존재착명현적혈관활성물질수평실형。
To investigate the relationship of vasoactive substance and cardiac func tion in patients with VVI pacing,the levels of plasma endothelin(ET)、atrial nat riuretic factor(ANP)、neuropeptide Y(NPY)、calictionin gene-related peptide(CGR P )、nerotensin(NT)and serum nitric oxide(NO)by radioimmunoassay and colorimetry were measured in 37 patients with VVI pacing before and after implantation 1,4 weeks and 30 healthy controls.At the same time,cardiac function and hemodynamic parameters were measured by echocardiography.Results:The levels of plasma ET,NPY,A NP,CGRP in patients with VVI pacing were significantly higher than those in heal thy controls.But the levels of plasma NT and NO were lower.Compared with vasoact ive substance before and after pacemaker implantation,the levels of plasma ET,CG RP didn′t changed.ANP,NPY got markly lower,and NT,NO got significantly higher In the no-pacemaker syndrome;the levels of plasma ET,ANP,CGRP,NPY,and serum NO increased significantly,the levels of plasma NT deceased in group of pacemaker sy ndrome.There was a positive correlation between mean pulmonary artery press and ET,ANP,CGRP,NPY,NO,and a negtive correlation between left,right ventricule eject fraction and ET,ANP,CRP,NPY,NO.Conclusions:The imbalance of vasoactive substa nce exists in the patients with pacemaker syndrome.
