目的研究我国苗族人群1型糖尿病(T1DM)患者与HLA-DR基因频率的相关性.方法选择无血缘关系的纯苗族T1DM患者45例,纯汉族T1DM患者43例,纯苗族健康对照(NC)组52例,纯汉族NC组58例.运用序列特异性引物-聚合酶链反应(PCR/SSP)技术测定HLA-DR1-10基因频率.结果(1)苗族及汉族T1DM组患者的DR2基因频率分别明显低于苗族及汉族NC组(P<0.05);DR3、DR9基因频率分别高于NC组(P<0.05).(2)苗族T1DM组及苗族NC组的DR5基因频率分别明显高于汉族T1DM组及汉族NC组(P<0.05).(3)苗族T1DM组的DR2基因频率明显低于汉族NC组(P<0.05);DR3、DR5、DR9基因频率分别高于汉族NC组(P<0.05).(4)苗族及汉族人群中,T1DM组的DR2基因频率分别明显低于NC组,其相对危险度RR<1,P<0.05;DR3、DR9基因频率分别高于NC组,其相对危险度RR>1,P<0.05.结论苗族T1DM组与汉族T1DM组患者HLA-DR1、DR2、DRa、DR3、DR6、DR7、DR8、DR9、DR10基因频率差异无统计学意义,苗族T1DM组及苗族NC组DR5基因频率高于汉族T1DM组及汉族NC组.DR3、DR9与T1DM呈正相关;DR2与T1DM呈负相关.

Objective To study the correlation between type 1 diabetes(T1DM) in Chinese Miao population and HLA-DR genetic frequency. Methods Forty five T1DM in unadulterated Miao population and 43 T1DM in unadulterated Han population were studied. The patients between the two groups had no kinship. Fifty two healthy unadulterated Miao population and 58 healthy unadulterated Han population were controls. The sequence-specific primer-polymerase chain reaction (PCR-SSP) techniques were adopted to test the frequencies of HLA-DR1-10. Results (1) DR2 frequency of T1DM in Miao and Han population was lower than that of the healthy Miao and Han population (P<0.05). Their DR3, DR9 frequencies were higher than those of healthy Miao and Han population respectively(all P<0.05). (2) DR5 frequencies of T1DM and healthy Miao population were higher than those of T1DM and healthy Han population(P<0.05). (3) DR2 genetic frequency of T1DM Miao population was lower than that of the healthy Han population (P<0.05). DR3,DR5,DR9 frequencies were respectively higher in T1DM Miao population than those of healthy Han population (P<0.05). (4) DR2 frequencies of T1DM Miao and Han population were lower than those of the healthy Miao and Han population with relative risk<1, P<0.05. Their DR3, DR9 frequencies were higher than those of healthy Miao and Han population respectively with relative risk >1, P<0.05. Conclusions There is no obvious difference in HLA-DR1,DR2,DR3,DR4,DR6,DR7,DR8,DR9,DR10 frequencies between T1DM Miao and Han population. DR5 frequencies of T1DM and healthy Miao population are higher than those of T1DM and healthy Han population. The DR3 and DR9 are in positive correlation with T1DM, the DR2 is in negative correlation with T1DM.
