目的 了解慢性阻塞性肺疾病(COPD)患者血清白蛋白水平与病情严重度分级的相关性.方法 120例COPD患者按病情严重度分级分为Ⅰ、Ⅱ、Ⅲ、Ⅳ四级,收集患者空腹血清白蛋白,采用微型营养评定表(MNA)、体质量指数(BMI)、血清白蛋白3个营养指标分别对患者进行营养评定,并分析比较.结果 Ⅰ级COPD患者营养良好者、轻度营养不良者和低白蛋白血症者人数分别为34例、6例、1例,Ⅱ级分别为8例、9例、2例,Ⅲ级分别为18例、12例、4例,Ⅳ级分别为7例、13例、6例,营养不良发生率分别为17.1%、57.9%、47.1%和73.1%;COPD严重度分级与血清白蛋白的相关指数为0.251 (P <0.05);COPD患者Ⅰ级与Ⅱ、Ⅲ、Ⅳ级间血清白蛋白的相关指数分别为0.273、0.331、0.324(均P<0.05).结论 COPD严重度分级与血清白蛋白的下降具有相关性,随COPD级别增加血清白蛋白下降越明显.
目的 瞭解慢性阻塞性肺疾病(COPD)患者血清白蛋白水平與病情嚴重度分級的相關性.方法 120例COPD患者按病情嚴重度分級分為Ⅰ、Ⅱ、Ⅲ、Ⅳ四級,收集患者空腹血清白蛋白,採用微型營養評定錶(MNA)、體質量指數(BMI)、血清白蛋白3箇營養指標分彆對患者進行營養評定,併分析比較.結果 Ⅰ級COPD患者營養良好者、輕度營養不良者和低白蛋白血癥者人數分彆為34例、6例、1例,Ⅱ級分彆為8例、9例、2例,Ⅲ級分彆為18例、12例、4例,Ⅳ級分彆為7例、13例、6例,營養不良髮生率分彆為17.1%、57.9%、47.1%和73.1%;COPD嚴重度分級與血清白蛋白的相關指數為0.251 (P <0.05);COPD患者Ⅰ級與Ⅱ、Ⅲ、Ⅳ級間血清白蛋白的相關指數分彆為0.273、0.331、0.324(均P<0.05).結論 COPD嚴重度分級與血清白蛋白的下降具有相關性,隨COPD級彆增加血清白蛋白下降越明顯.
목적 료해만성조새성폐질병(COPD)환자혈청백단백수평여병정엄중도분급적상관성.방법 120례COPD환자안병정엄중도분급분위Ⅰ、Ⅱ、Ⅲ、Ⅳ사급,수집환자공복혈청백단백,채용미형영양평정표(MNA)、체질량지수(BMI)、혈청백단백3개영양지표분별대환자진행영양평정,병분석비교.결과 Ⅰ급COPD환자영양량호자、경도영양불량자화저백단백혈증자인수분별위34례、6례、1례,Ⅱ급분별위8례、9례、2례,Ⅲ급분별위18례、12례、4례,Ⅳ급분별위7례、13례、6례,영양불량발생솔분별위17.1%、57.9%、47.1%화73.1%;COPD엄중도분급여혈청백단백적상관지수위0.251 (P <0.05);COPD환자Ⅰ급여Ⅱ、Ⅲ、Ⅳ급간혈청백단백적상관지수분별위0.273、0.331、0.324(균P<0.05).결론 COPD엄중도분급여혈청백단백적하강구유상관성,수COPD급별증가혈청백단백하강월명현.
Objective To explore serum albumin level of the patients with chronic obstructive pulmonary disease(COPD) and disease severity grading of the relationship between the early,better,a reasonable nutritional support in patients with COPD.Methods 120 COPD patients were randomly divided into four levels according to the disease severity,Ⅰ level as the control group,Ⅱ,Ⅲ,Ⅳ grade for the experimental group,collectting fasting serum albumin,micro-nutrition assessment form ( MNA),body mass index ( BMI ),serum albumin,three nutrition indicators were assessed in patients with COPD,the control group data were compared with the three experimental groups.Results Ⅰ level of COPD were well-nourished patients,mild malnutrition and hypoalbuminemia were 34,6,1 respectively.Ⅱ level were 8,9,2,Ⅲ level were 18,12,4,Ⅳ level were 7,13,6.Malnutrition rates were 17.1%,57.9%,47.1% and 73.1%,COPD associated with serum albumin level index was 0.251 (P < 0.05 ) ;COPD patients with grade Ⅰ and Ⅱ,Ⅲ,Ⅳ level of serum white protein-related indices were 0.273,0.331,0.324 ( all P < 0.05 ).Conclusion COPD severity grading and the decline in serum albumin were correlated,the serum albumin level decreased more significantly.
