目的 探讨三维CT重建在累及关节面的复杂骨折治疗中的指导意义.方法 包钢第三职工医院自2008年至今共收治累及关节面的复杂骨折40例(髋臼骨折12例,胫骨平台骨折18例,踝关节骨折10例),术前均行X线摄片并行CT扫描和三维CT重建,对关节面损伤程度进行评估并制定最佳治疗方案.结果 12例髋臼骨折中9例行手术治疗,18例胫骨平台骨折中16例行手术治疗,10例踝关节骨折中7例行手术治疗.结论 三维重建成像能立体地、直观地显示传统X线摄片所不能直接显示的关节面骨折,尤其在显示隐匿性骨折线方面明显优于CT平扫,对手术前的骨折分类、治疗方案制订以及手术入路选择有一定的指导意义,能为制定精细的手术方案提供可靠依据.
目的 探討三維CT重建在纍及關節麵的複雜骨摺治療中的指導意義.方法 包鋼第三職工醫院自2008年至今共收治纍及關節麵的複雜骨摺40例(髖臼骨摺12例,脛骨平檯骨摺18例,踝關節骨摺10例),術前均行X線攝片併行CT掃描和三維CT重建,對關節麵損傷程度進行評估併製定最佳治療方案.結果 12例髖臼骨摺中9例行手術治療,18例脛骨平檯骨摺中16例行手術治療,10例踝關節骨摺中7例行手術治療.結論 三維重建成像能立體地、直觀地顯示傳統X線攝片所不能直接顯示的關節麵骨摺,尤其在顯示隱匿性骨摺線方麵明顯優于CT平掃,對手術前的骨摺分類、治療方案製訂以及手術入路選擇有一定的指導意義,能為製定精細的手術方案提供可靠依據.
목적 탐토삼유CT중건재루급관절면적복잡골절치료중적지도의의.방법 포강제삼직공의원자2008년지금공수치루급관절면적복잡골절40례(관구골절12례,경골평태골절18례,과관절골절10례),술전균행X선섭편병행CT소묘화삼유CT중건,대관절면손상정도진행평고병제정최가치료방안.결과 12례관구골절중9례행수술치료,18례경골평태골절중16례행수술치료,10례과관절골절중7례행수술치료.결론 삼유중건성상능입체지、직관지현시전통X선섭편소불능직접현시적관절면골절,우기재현시은닉성골절선방면명현우우CT평소,대수술전적골절분류、치료방안제정이급수술입로선택유일정적지도의의,능위제정정세적수술방안제공가고의거.
[Objective] To investigate the guiding significance of three-dimensional CT reconstruction in treatment of complex fractures involving the articular surface.[Methods] From 2008 to the present,40 patients with comlex fractures involving the articular surface(acetabular fractures in 12 cases,tibial plateau fractures in 18 cases,ankle fractures in 10 cases)were admitted,preoperative X-ray,CT scanning and three-dimensional CT reconstruction of the articular surface were performed to assess the degree of injury and make best treatment options.[Results] Nine cases out of 12 cases of acetabular fracture underwent surgery treatment,16 cases out of 18 cases of tibial plateau fractures underwent surgical treatment,7 cases out of 10 cases of ankle fractures underwent surgical treatment.[Conclusions] Three-dimensional reconstruction images can three-dimensionally,visually display the articular surface fractures which traditional X-ray can not directly show,especially is superior to CT scan in the show of occult fracture line,and has some guiding significance in classification of fracture before surgery,treatment options formulation and the choice of surgical approach.Therefore,it can provide reliable basis for making fine surgical method.
