目的 研究电磁波源体外冲击波碎石机碎石治疗对肾脏的急性损害.方法 检测30例行电磁波源体外冲击波碎石治疗患者的尿酶[N-乙酰-β-氨基葡萄糖苷酶(NAG)、γ-谷氨酰胺转移酶(γ-GT)]和尿β2微球蛋白(β2-MG)水平并进行对比.结果 患者体外冲击波碎石2 h后尿NAG、γ-GT均明显升高[(19.92±8.16)比(4.61±1.76)、(96.25±32.12)比(28.89±7.62),P<0.01],7 d左右恢复至治疗前水平[(4.96±1.88)、(31.35±7.65)];患者碎石后尿β2-MG明显升高,在第7天时有轻微的升高,第15天降至正常.结论 电磁波源体外冲击波碎石治疗会引起急性肾功能损害.

목적 연구전자파원체외충격파쇄석궤쇄석치료대신장적급성손해.방법 검측30례행전자파원체외충격파쇄석치료환자적뇨매[N-을선-β-안기포도당감매(NAG)、γ-곡안선알전이매(γ-GT)]화뇨β2미구단백(β2-MG)수평병진행대비.결과 환자체외충격파쇄석2 h후뇨NAG、γ-GT균명현승고[(19.92±8.16)비(4.61±1.76)、(96.25±32.12)비(28.89±7.62),P<0.01],7 d좌우회복지치료전수평[(4.96±1.88)、(31.35±7.65)];환자쇄석후뇨β2-MG명현승고,재제7천시유경미적승고,제15천강지정상.결론 전자파원체외충격파쇄석치료회인기급성신공능손해.
Objective To understand the acute renal function damage after treatment of electromechanical shock wave lithotripsy. Methods Patients received the treatment of electromechanical model. The short term effect of shock wave energy was studied by measuring urinary enzyme activity (NAG;γ-GT)and the excretion of β2-MG.Results Sharply elevated NAG and γ-GT were detected after ESWL and returned to baseline by day 7. β2-MG increased remarkably after ESWL. On day 7 only a mild but still significant elevation was noted. Excretion decreased to baseline by day 15. Conclusions Treatment of electromechanical device may cause acute renal function damage. Significant differences are related to concentration of enzymuria.
