目的 探讨东莨菪碱联合呼气末正压通气模式治疗重度急性有机磷农药中毒患者的临床疗效.方法 将76例重度急性有机磷重度患者完全随机分为2组,每组38例,治疗组采用东莨菪碱联合呼气末正压通气模式治疗,对照组采用传统方法治疗.比较2组患者的动态检测胆碱酯酶活性(CHE),观察东莨菪碱用量、昏迷时间、机械通气时间、病死率、住院时间及并发症发生率.结果 治疗组治愈30例(80%),较对照组26例(68.4%)高;治疗组病死4例(10.5%),较对照组10例(26.3%)低;2组差异均有统计学意义(均P<0.05);与对照组比较,治疗组CHE恢复时间、昏迷时间、机械通气时间缩短,并发症发生率和病死率明显降低(P<0.05).结论 东莨菪碱联合呼气末正压通气模式治疗重度急性有机磷农药中毒疗效确切,临床可推广应用.

Objective To observe the therapeutic effect of positive end expiratory pressure combined with scopolamine on severe acute organophosphorus pesticide poisoning. Methods Thirty-eight patients with severe acute organophosphorus pesticide poisoning were treated by scopolamine combined with positive end expiratory pressure(treatment group). 38 patients with severe acute organo phosphorus pesticide poisoning treated by routine therapy(control group) were also enrolled. The activity of choline esterase (CHE), dosage of ponehyclidine hydrochloride, duration of coma, time of mechanicaiventilation, mortality, hospitalization time and incidence of complications were detecte. Results Compared with control group, the activity of CHE, duration of coma, time of mechanical ventilation, mortality and incidence of complication in treatment group were significantly decreased (P< 0.05). Conclusion Scopolamine combined with positive end expiratory pressure has a definite therapeutic effect on severe acute organophosphorus pesticide poisoning.
