目的评价银杏达莫联合甲钴胺治疗对糖尿病心脏自主神经病变患者心率变异性的影响.方法对66例糖尿病心脏自主神经病变患者随机分为治疗组(n=33)和对照组(n=33),治疗组患者给予银杏达莫20 ml加入生理盐水250 ml静滴,联合甲钴胺0.5mg加牛理盐水10ml静注,每日1次,总疗程3周.对照组给予肌注维生素B1和维牛素B12,每日1次.运用24小时动态心电图榆测所有患者治疗前后心率变异性指标,观察治疗前后上述指标的变化,并进行对比分析.结果治疗组治疗后心率变异性各项指标较治疗前有显著提高,自主神经病变症状明显好转,与对照组比较差异有显著性(P＜0.05).结论银杏达莫联合甲钴胺治疗糖尿病心脏自主神经病变的疗效显著,可以改善患者心率变异性.

Objective To evaluate the effect of Ginkgo leaf extract and dipyridamole injection combined with methylcobalamin treatment to heart rate variability of patients with diabetic cardiac autonomic neuropathy. Methods 66 patients of diabetic cardiac autonomic neuropathy were randomly divided into treatment group and control group, 33 cases of each group, the treatment group were treated with normal saline 250 ml plus Ginkgo leaf extract and dipyridamole injection 20 ml intravenous infusion,and normal saline 10 ml plus methylcobalarnmin 0.5 mg intravenousry once a day,with a total course of treatment for3 weeks. Control group was injected with vitamin B1 and vitamin B12, once a day. 24-hour ambulatory ECG testing was used to collect heart rate variability index of all patients before and after treatment, changes in these indicators were also observed and analyzed. Results After treatment, the heart rate variability index was significantly increased compared with before treatment, and autonomic neuropathy symptoms improved, compared with the control group, the differences was statically significant (P＜0.05). Conclusion Ginkgo leaf extract and dipyridamole injection combined with methylcobalamin treatment of diabetic cardiac autonomic neuropathy has significant effect and can improve heart rate variability.
