目的 探讨椎弓根螺钉技术在胸椎骨折中的应用.方法 对74例胸椎骨折患者进行回顾性分析,74例患者皆行手术治疗,其中单纯后路减压者11例,减压及行椎弓根钉内固定者63例,根据骨折不同的部位,分别选用Dick钉、AF钉和TSRH钉内固定.结果 74例患者手术切口全部Ⅰ期愈合,无胸膜损伤、截瘫加重、大血管损伤等并发症,15例患者出现脑脊液漏,经治疗后痊愈.术后截瘫症状未见明显恢复.结论 后路减压及椎弓根钉内固定治疗胸椎骨折,能有效地恢复脊柱的正常序列,有利于患者早期进行康复锻炼,但对神经功能的恢复无明显影响.

목적 탐토추궁근라정기술재흉추골절중적응용.방법 대74례흉추골절환자진행회고성분석,74례환자개행수술치료,기중단순후로감압자11례,감압급행추궁근정내고정자63례,근거골절불동적부위,분별선용Dick정、AF정화TSRH정내고정.결과 74례환자수술절구전부Ⅰ기유합,무흉막손상、절탄가중、대혈관손상등병발증,15례환자출현뇌척액루,경치료후전유.술후절탄증상미견명현회복.결론 후로감압급추궁근정내고정치료흉추골절,능유효지회복척주적정상서렬,유리우환자조기진행강복단련,단대신경공능적회복무명현영향.
Objective To investigate the usage of the internal fixation technique with the pedicle of vertebral arch nail in the fracture of thoracic vertebrae.Methods All 74 patients with the fracture of thoracic vertebrae were retrospectively analyzed. All 74 patients had the operative treatment ; 11 had the simple posterior decompression and other 63 patients had the decompression with the internal fixation of the pedicle of vertebral arch nail.According to the different fracture section,we used dick nail,atlas fixator nail and vertebral pedicle screw nail separately.Results All of the 74 patients were cured primarily.Nobody had complications of the injury of the pleura,the exacerbation of paraplegia and the injury of the big blood vessels.Fifteen patients had the leakage of cerebrospinal but were cured after treatment. The symptom of paralysis seldom recovered postoperatively.Conclusion Posterior decompression and pedicle screw fixation can effectively restore the spinal normal sequence,which is beneficial to patients with early rehabilitation exercise,but it has an effect on the recovery of the nervous function.
