目的探讨Gamma3钉与股骨近端防旋髓内钉(PFNA)治疗不稳定性股骨粗隆间骨折的临床疗效.方法选择2006年1月至2010年9月于湖北省中山医院骨外科行手术治疗的股骨粗隆间骨折患者45例,分别行PFNA(PFNA组,23例)及Gamma3钉(Gamma3钉组,22例)治疗.采用髋关节功能Harris评分系统评定对两组平均手术时间、平均手术失血量度骨折愈合时间等进行比较.结果术后随访6-18个月,平均12,6个月,所有患者患髋功能恢复良好.PFNA组与Gamma3钉组间比较差异有统计学意义(P＜0.05).结论 PFNA治疗具有手术时间短,固定牢固,术中损伤小、出血少,术后并发症少等特点,临床疗效优于Gamma3钉,是治疗股骨粗隆间骨折的理想内固定物.

Objective To investigate the clinical effect of Gamma3 nail and proximal femoral nail antirotation( PFNA ) on unstable femoral intertrochanteric fractures.Methods From January 2006 to September 2010,45 cases of femoral intertrochanteric fractures underwent surgery treatment in our department were choosed.Twenty-two cases were fixed with Gamma3 nail and 23 cases with PFNA.According to the Harris score system,the operation time,the amount of intraoperative blood loss and the fracture healing time were compared between the two groups.Results The patients were followed up for 6 to 18 months( average 12.6 months).All patients succeeded in operation with fair and good hip joint function.There were significant differences between the two groups (P ＜ 0.05).Conclusions PFNA treatment has advantages of short operative time,firm fixation,less bleeding,minimal invasive and less postoperative complications,the clinical effect is superior to Gamma3 nail.So it is a better choice for treatment of femoral intertrochanteric fractures.
