目的 探讨心力衰竭(简称心衰)患者血清瘦素水平、胰岛素抵抗(IR)与心衰严重程度的关系.方法 选择心衰患者90例,按NYHA心功能分级分为三组(心功能Ⅱ级、心功能Ⅲ级、心功能Ⅳ级组),每组30例.健康对照组60例,放免法测定血清瘦素、空腹胰岛素(FINS)水平,全自动生化分析仪测定空腹血糖(FBG)水平,并计算胰岛素抵抗指数(HOMA-IR).结果 心衰组血清瘦素、胰岛素抵抗水平较对照组显著增高(P<0.01);心衰组瘦素,胰岛素抵抗水平按NYHA分级分别为:NYHA分级Ⅱ级为(8.17±4.39)ng/L、1.98±0.96,NYHA分级Ⅲ级为(11.28 ±4.95)ng/L、3.48±1.26,NYHA分级Ⅳ级为(14.29±5.85)ng/L、5.32±1.52,差异有统计学意义(P<0.05).结论 心衰患者血清瘦素、胰岛素抵抗水平显著升高,并与心衰严重程度存在一定相关性,可以较准确地评价患者心衰的严重程度.

목적 탐토심력쇠갈(간칭심쇠)환자혈청수소수평、이도소저항(IR)여심쇠엄중정도적관계.방법 선택심쇠환자90례,안NYHA심공능분급분위삼조(심공능Ⅱ급、심공능Ⅲ급、심공능Ⅳ급조),매조30례.건강대조조60례,방면법측정혈청수소、공복이도소(FINS)수평,전자동생화분석의측정공복혈당(FBG)수평,병계산이도소저항지수(HOMA-IR).결과 심쇠조혈청수소、이도소저항수평교대조조현저증고(P<0.01);심쇠조수소,이도소저항수평안NYHA분급분별위:NYHA분급Ⅱ급위(8.17±4.39)ng/L、1.98±0.96,NYHA분급Ⅲ급위(11.28 ±4.95)ng/L、3.48±1.26,NYHA분급Ⅳ급위(14.29±5.85)ng/L、5.32±1.52,차이유통계학의의(P<0.05).결론 심쇠환자혈청수소、이도소저항수평현저승고,병여심쇠엄중정도존재일정상관성,가이교준학지평개환자심쇠적엄중정도.
Objective To explore the relations between serum leptin and insulin resistance (IR)and the degree of heart failure. Methods 150subjects were selected, including 60 healthy controls and 90 patients with heart failure. We determined the concentration of fasting insulin (FINS)and leptin in serum in these patients by radioimmunoassay. Insulin resistance index was calculated to evaluate insulin resistance degree. Results The concentrations of leptin and HOMA-IR in heart failure cases were significant higher than those in the control group (P < 0.01 )and it was also showed that the more severe the heart failure, the higher the leptin and HOMAIR[(8.17 ± 4.39) ng/L, ( 1.98 ± 0.96) in class Ⅱ, ( 11.28 ± 4.95) ng/L, (3.48 ± 1.26) in class Ⅲ, ( 14.29± 5.85 ) ng/L, (5.32 ± 1.52) in class Ⅳ, ( P < 0.05 )]. Conclusion The serum leptin concentrations and degree of insulin resistance were significantly higher in patients with heart failure, they are correlative with the degree of heart failure, and may evaluate the severe degree of heart failure.
