目的 研究外源性血管内皮生长因子(VEGF)对大鼠脑出血后血肿周围区新生血管、GAP - 43表达及神经功能恢复的影响,探讨外源性VEGF对脑出血的保护机制.方法 采用二步自体血脑内注入法建立脑出血动物模型.36只雄性SD大鼠随机分为假手术组、模型组、治疗组.治疗组于术后8-15 d连续经腹腔给予外源性VEGF,给药后15d和30 d分别采用免疫组化法检测CD34、GAP -43的表达.用CD34的表达来反映血管新生情况,用GAP - 43、Longa神经功能评分反映神经损伤及修复情况.结果 治疗组CD34阳性微血管密度及血管场面积在15 d和30 d均高于模型组及假手术组(P<0.01).治疗组15 d及30 d的GAP - 43阳性细胞表达率及阳性颗粒所占的面积比上调,与模型组、假手术组比较差异有统计学意义(P<0.01),Longa评分提示治疗组神经功能恢复提前于模型组(P<0.01).结论 外源性VEGF可促进新生血管生长,提高受损中枢神经系统GAP- 43表达,加快神经功能的恢复.

Objective To study the effects of exogenous VEGF on cerebral angiogenesis,the expression of GAP-43 and nerve functional recovery in rat with intracerebral hemorrhage (ICH),to reveal the mechanism of neuroprotection by exogcnous VEGF.Methods The model of ICH was established in rats by two - step intracerebral injection of autogenous blood.36 male Spranue - Dawley rats were randomly divided into sham- operated group,ICH model group and treatment group.The treatment group was inject with VEGF intraperitoneally during postoperative Day8 to Day15.The expression of CD34,GAP- 43 were detected by immunohistochemistry on Day15 and Day30.Microvascular production was measured by expression changes of CD34.Neurotrosis and neural Repair was measured by expression changes of GAP -34 and Longa evaluation.Results the expression of CD34 + microvascular density and vascular field area of treatment group in Day15 and Day30 are significantly higher than those of model group and sham -operated group ( P < 0.01 ).The rate of positive cell of GAP -43 and Positive particles area of the treatment group in 15 d and 30 d are significantly increased,compared to the model group and the sham - operated group.Longa evaluation showed significant difference among model group and treatment group in Day15.Longa evaluation of treatment group was better than thoes of model group.Conclusions Exogenous VEGF could enhance angiogenesis and up - regulate the expressions of GAP - 43 after intracerebral hemorrhage.It could reduce Neurotrosis caused by intracerebral hemorrhage and improve neurological function.
